BASIC PURPOSE
The purpose of the Registered Dietician is to support the Medically Tailored Meals team with nutrition planning, program development and quality assurance.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Conduct intake and program eligibility for Medically Tailored Meals services
Conduct nutrition assessments
Develop and implement individualized nutrition plans
Review and monitor all meal and/or grocery plans for Medically Tailored Meals
Ensure menu options are equally nutritious and culturally appropriate
Plan and conduct routine nutrition workshops and trainings
Coordinate with others agencies and providers to ensure continuity of care
Coordinate medical referrals in coordination with contracted managed care plans
Participate in weekly case conference meetings
Participate in program and agency trainings as assigned
Maintain accurate participant records in various information management systems; and generate reports as requested
Maintain and execute confidential information according to HIPPA standards
Maintain a highly detailed and organized filling system
Ensure intake and care procedures utilize harm reduction and trauma informed care principles
Check and respond to emails and voicemails on a regular basis
Adhere to confidentiality standards
Other duties as assigned
KNOWLEDGE, SKILLS, ABILITIES AND OTHER QUALIFICATIONS REQUIREMENTS
Ability to speak and write the English language at a high and professional level
High degree of confidentiality
Able to endorse and promote The Salvation Army’s mission
Computer literate, in Windows environment (Microsoft Office), Word, Publisher, Excel and Outlook preferred
Excellent communication skills, both written and verbal.
Excellent and professional telephone etiquette and presence
Excellent organizational skills
Strong ability to utilize a high level of time management and handling multiple tasks
CERTIFICATES, LICENSES, REGISTRATIONS
Bachelor's degree in Dietetics, Food and Nutrition, or related field.
Credentialed as a Registered Dietitian by the Commission on Dietetic Registration (CDR), required
Must possess food handler’s certification
Must possess a valid California Class C Driver License, and ability to drive a Salvation Army vehicle
Must be 21 years or older
Authorize The Salvation Army to add name to the CA DMV Pull Notice Program
Complete The Salvation Army vehicle course training
PHYSICAL REQUIREMENTS:
Ability to sit, walk, stand, bend, squat, climb, kneel, and twist on an intermittent or sometimes continuous basis
Ability to grasp, push, and/or pull objects
Ability to reach overhead
Ability to operate telephone
Ability to lift up to 25-40 lbs.
Ability to operate a computer
Ability to process written, visual, and/or verbal information
Ability to operate basic office equipment and tools PC, Fax Machine, Telephone, Calculator, Copier, Printer.
