Leake and Watts Services Intake and Recreation Coordinator in Yonkers, New York

The Unaccompanied Children Program provides long term and transitional residential care and other supportive services to Unaccompanied Children (UC) who are under legal custody of the Federal Government. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in a Basic Shelter program, and the Therapeutic Group Home programs located at our licensed residential facility in Yonkers, NY as well as a Transitional Foster Care program located in foster homes in the Bronx, Queens and Westchester. In all programs, we provide therapeutic and educational services and other supports that youth need to achieve success through an array of strength-based and targeted interventions as well as link them with family members and/or other sponsors in the United States so they are able to move forward with their lives and thrive.

Upon admission

  • Responsible for the completion of inventory of all items that are provided to minors at the time of arrival. (personal hygiene products)

  • Ensure that staff completes the inventory at admission and that any missing items are available to the child on the next business day or as soon as possible.

  • Ensure that Intake/Discharge inventories are placed in the child’s file and intake file.

  • Ensure that all items must be locked at all times

  • Update of the intake inventory while the child is in care.

  • Ensure the inventory is provided to the assigned CM upon release of the child.

  • Create child’s file (blue) with label, record file index, NTA packet, minor’s picture, signed initial intake assessment, trigger reports, orientation packet, intake inventory and distribution forms.

  • Responsible for filing and closing out closed cases.

  • Coordinating and reporting of daily census for the program sent to Administrative staff, ORR, ICE and Catholic Charities of NY.

Lewis and ORR portal.

  • Admit children in the system (within 24 hours of admission)

  • Discharge children from portal as requested

  • (Lewis) Reflect children’s movement as needed: discharges, transfers and hospitalizations

  • Email CM and LCM with request for child movement form once travel discharge email has been received

  • Send email to Assistant Director of Foster Care when movements are completed.

Portal Intake Assessment

  • Intake Assessment has to be completed with the minor and in the portal within 24 hours of admission.

  • Print and sign Intake Assessment (place in child’s file)

  • Take a picture of the child, upload to the Portal (place picture with label in child’s file)

  • Print Trigger reports (Medical and Authorization of Placement) and bring them to the Program Director to be signed

  • Send an email to POH informing of the completion of the Intake

  • For additional confidential information obtained during the intake, an email must be sent to the assigned Clinician, Case Manager Supervisors, Lead Clinician, Assistant Director of Clinical and Case Management, Assistant Director of Foster Care, Assistant Director of residential and Program Director.

  • Within 48 hours of admission, complete the Orientation packet with the children and provide one copy of the signed packet to the Case Manager and one to the child.

  • Ensure that the approved list of contacts of the children is sent to the team within 24 hours of admission

Transitional Foster Care Program

  • Responsible for weekly tracking of the following documents given to the Foster Parents

  • Weekly Phone cards

  • Phone Call Log

  • Recreational and Spiritual Logs

  • Call Foster Parents on 1st and 2nd day of child’s placement to ensure receipt of Contact Folder and informational forms.

  • Based on the approved Phone Contact List sent by the Case Manager, Complete the form sent to the Foster Parents, This must be completed within 48 hours of the child’s placement to the program.

  • Daily review of the completion of Daily Sign-in sheet for drop offs and pickups of each minor.

  • Upon the child’s approval for release from the program. A follow up call must be made to the Foster Parent to remind them to send all pending forms or documentation.

Orientation Process:

  • Complete Orientation sessions with all new minors within 48 hours of admission to the program.

  • Upon completion of the orientation session, update the Orientation tracker spreadsheet in the POH O Drive

  • Send an email to POH informing that Orientation took place and include the date and name of all the minors.

  • Place Orientation packet in the minor’s file (blue) prior to giving file to Case Managers.

  • Ensure to complete 90 day orientation sessions with all minors in program and track orientations in o-drive (create folder)

  • Discuss with the children program expectations, Children’s Rights and Responsibilities, Grievance Letter and Values. This will be completed in conjunction with the Program Director.

  • Ensue that this information is given to the CM for filing.

  • Ensure that the children get a copy of this information

  • Introduction to Behavioral Plan and PBIS (Gotchas and Check-in)

  • Distribute the approval contact information within a day for all children in the program. Follow-up with each Case Manager to ensure that the information for each new minor is sent within 24 hours of admission to the program.

Recreational Program:

  • Create monthly Recreational Calendar and distributing it monthly

  • Supervision to staff recreational facilitator who assist in creating calendar of activities.

  • Ensure necessary supplies are available for activities listed.

  • Ensure that recreational program is being executed

  • Ensure that calendar is posted and updated.

  • Weekly rounds to ensure compliance and identify any barrier for success.

  • Meet on a Weekly or bi-weekly basis with Recreational Team (both staff and children)

  • Maintain weekly review of Binders (Recreational, Spiritual) for appropriate completion of information.

  • Associates Degree; Bachelor Degree Preferred in a Human Services

  • Bi-Lingual (Spanish/English) a must

  • Excellent verbal, written communication and organizational skills

  • Proficient in Microsoft (Excel, Word, PowerPoint)
