JOB TITLE:                  Case Manager

DEPARTMENT:          Supportive Housing

FLSA:                           Non-Exempt

LOCATION:                  Yonkers, NY

REPORTS TO:              Day Spring Commons Service Coordinator

SALARY:                       $40,000.00 to $45000.00

 

SUMMARY:  

 Ensures that all clients enrolled in the program are assessed and referred for the appropriate services . Provide Case management and supportive services to Young Adult permanent residents and their children who reside at Day Spring Common. Attends all necessary program meetings scheduled with collateral agencies. Proof of COVID-19 vaccinations required.

DUTIES AND RESPONSIBILITIES:

  • Responsible for implementing service plans, reassessments and progress notes.

  • Complete intakes on each family member and processing of new move-ins

  • Act as a liaison between families and the Department of Social Services.

  • Provide crisis intervention services as needed.

  • Meet with residents to assist with development of treatment, vocational training, child care/afterschool programs and continuing education

  • Assists with school transportation, registration, and monitor school attendance.

  • Organize and co-facilitate client groups.

  • Maintain accurate, complete and current files on all clients.

  • Maintains contact with all necessary collaterals and serves as a liaison between clients and community agencies.

  • Conducts monthly unit inspections or as needed.

  • Accompany clients on appointments as needed.

  • Reports all property/unit deficiencies to Service Coordinator

  • Assists with the monitoring of monthly rent payments, delinquent accounts and appropriate follow-ups under the direction of the Service Coordinator

  • Other duties as assigned

    EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS:  

    Bachelor degree required. Two years of related and/or applicable experience working with children and families. Fluency in Spanish is highly desirable. Ability to work evenings and occasional weekends, if needed. Excellent and demonstrated writing ability with strong spelling, grammatical and punctuation skills, and strong oral communication skills. Advanced knowledge of MS Word, Excel, PowerPoint and database management skills.
