BayCare Health System is currently in search of our newest Hospital Care Coordinator who is passionate about providing outstanding customer service to our community at Winter Haven Hospital .
Hospital Care Coordinator - RN Case Management responsibilities:
Provides linkage, monitoring, planning and advocacy for our population to achieve and maintain maximum functioning
Possesses a clear understanding of the available resources to promote quality cost effective outcomes
Effectively maintains and manages assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the individual served
Coordinates discharge planning efforts through assessment, referral, and interdisciplinary planning
Ability to adjust to changing priorities in a fast-paced environment

Qualifications:
Certifications and Licensures:
RN (Registered Nurse)
ACM (Case Management) Certification
CCM (Case Manager) Certification
Education:
Associate's Nursing
Bachelor's Nursing
Required experience:
2 years Registered Nurse
2 years Acute Care
Schedule: Monday, Tuesday, Wednesday, Thursday, Friday
Shift: Days
Shift Hours: 8:00 AM -4:30 PM
Weekend Work: Occasional
On Call: No
