Behavioral Health Case Mgr I – North Carolina
*MUST HAVE A CLINICAL LICENSE IN THE STATE OF NORTH CAROLINA.
Location: Hybrid position including virtual, community field visits with members and may occasionally need to report to a corporate office.
Residency in one of the following counties in the state of North Carolina is preferred but not required:
Bladen, Brunswick, Columbus, Cumberland, Harnett, Hoke, Lee, Montgomery, Moore, New Hanover, Pender, Richmond, Robeson, Sampson, Scotland, Avery, Buncombe, Burke, Caldwell, Cherokee, Clay, Graham, Haywood, Henderson, Jackson, Macon, Madison, McDowell, Mitchell, Polk, Rutherford, Swain, Transylvania, Yancey.
Work Shift: Monday-Friday: 8am-5pm (Eastern Standard Time)
The Behavioral Health Case Mgr I is responsible for performing case management telephonically and/or community field visits within the scope of licensure for members with behavioral health and substance abuse or substance abuse disorder needs. This position will be working with the Medicaid line of business.
How you will make an impact:
Use appropriate screening criteria knowledge and clinical judgment to assess member needs.
Conduct assessments to identify individual needs and develops care plan to address objectives and goals as identified during assessment.
Monitor and evaluate effectiveness of care plan and modifies plan as needed.
Support member access to appropriate quality and cost-effective care.
Coordinate with internal and external resources to meet identified needs of the members and collaborates with providers.
Minimum Requirements:
Requires MA/MS in social work, counseling, or a related behavioral health field or a degree in nursing, and minimum of 3 years clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment ; or any combination of education and experience which would provide an equivalent background.
Current, active, unrestricted license such as either a LCSW, LCMHC, LMFT, RN (must have behavioral health experience) or Clinical Psychologist to practice as a health professional within the scope of licensure in the state of North Carolina .
Preferred Skills, Capabilities and Experiences:
Experience in case management and telephonic and/or in person coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders preferred.
Bilingual communication skills in English and Spanish preferred.
