Winona Health Emergency Department Care Coordinator in Winona, Minnesota
1.0 FTE, 80 Hours a Pay Period Days, Hours between 8 am - 5 pm Weekends: No Holidays: No Position Overview: The Emergency Department Care Coordinator provides support, assessment, resources, interventions, and other services to patients and their families/support systems in the Emergency Department at Winona Health. Collaborates with the interdisciplinary health care staff to treat and plan for the social and medical needs of Emergency Department patient(s)/support systems. Devises, develops, and implements discharge plans as necessary. Assesses patients to evaluate social, emotional, and physical needs by applying appropriate theories and techniques. The Care Coordinator strives to promote patient and family wellness, improved care outcomes, and access to appropriate hospital and community resources among a patient population with complex health needs. Essential Duties & Responsibilities: Formulates recommended goals and interventions for high-risk patients in the Emergency Department. Provides interventions such as psychoeducation, skills training, crisis intervention, and problem-solving, while also offering social support and connection to externally available services to support needs. Develops goal-oriented discharge plans with patients to reduce the effects of chronic conditions and/or co-occurring disorders, such as mental health and chemical dependency issues. Documents all such interventions in the patient medical record as appropriate. Provides support, crisis intervention, and other appropriate therapeutic services to assist patients and families in coping with presenting symptoms and problems, accepting medical recommendations, and change in function/ability, etc. Provides education to patients and families around issues related to adaptation to the patient s diagnosis, illness, treatment, and/or life situation. Advocates for clients and family members within the medical system and the community to help with financial resources, housing, medical care, and other services. Participates in monitoring and evaluating the quality and appropriateness of patient care. Assists in providing resources for appropriate levels of care for patients including referrals and connections with Primary Care Services as needed. Direct resource for the Primary Care team members working toward value-based purchasing initiatives and strategies. Participates in and supports continuous improvement both in the Emergency Department and throughout the organization. Strives to eliminate waste and duplication and develops workflow(s) accordingly. Knowledge of crisis intervention, motivational interviewing, advance care planning, advance directive facilitation, and advanced communication skills are required. Valued member of the interdisciplinary team that coordinates and follows through on patient care needs including social, emotional, and medical needs. Interacts continually with medical staff in the Emergency Department and works side-by-side with nursing staff in the treatment plan for patients and their families. Assisting staff with patient needs as needed and performs other duties as assigned. Supervisory Responsibilities: No direct reports
