Case Manager

Position Type -Full Time

Job Description:

Listed below are the duties, projects and responsibilities of a CASE MANAGER. These duties include but not limited to conducting Client Intakes, Needs Assessments, Service Planning, Monitoring and Evaluation of assigned cases/clients.

  1. Interview individuals and families, complete intake, screening and assessment to obtain and exchange general and sensitive information, for the purpose of determining client need and service delivery.
  2. Demonstrate proficiency in independently managing a diverse caseload.
    1. Retrieve case summary through automated system (IBM Software) or logs as may be applicable.
    2. Analyze cases and determine next action step.
    3. Review and monitor case.
    4. Have contact with community partners/agencies concerning cases.
    5. Provide accurate information via telephone, in-person contact or email.
    6. Make necessary referrals for client services.
    7. Document all case activities.
  3. Perform monthly needs assessment to ensure clients are referred to appropriate services.
  4. Promote self-sufficiency and reduce dependency:
    1. Communicate and interact with clients in accordance with the DHSS and DSS Mission and Vision statements as well as the DHSS Beliefs and Principles.
    2. Educate clients to allow them to make informed choices regarding services.
    3. Develop rapport with clients, using skills identified in applicable trainings.
  5. Coordinate work activities with the internal and external partners.
  6. Demonstrate knowledge of financial coaching and the ability to determine when a referral is necessary.
  7. Complete and review reports and work assignments for completeness and accuracy within specified time frames.
  8. Process documents for and through the Document Imaging System (DIS) via indexing.
  9. Link staff, clients and other agencies to available resources.
  10. Use case management techniques to ensure clients are meeting or exceeding goals.
  11. Participate on task forces, special projects, committees and outreach activities as needed.
  12. Complete refresher courses and all required/assigned courses and tests by designated deadlines in DLC.
  13. Attend and participate in all required meetings and training workshops.
  14. Perform other duties as assigned by supervisor.

Qualifications

Job Requirements:

  1. Experience with Outlook, Microsoft Office Suite, and Google Drive and Docs.
  2. Experience in customer service.
  3. Experience in handling confidential information.
  4. Experience in coaching clients on life skills.
  5. Experience with case management or similar fields.
