Our Position: We are currently seeking a Coordinator of Day Habilitation to develop, coordinate and supervise the service activities in Group Day Habilitation and approves all assessments, service plans and monthly notes, assures compliance with Agency standards and guidelines.

This position also involves:

  • Monitoring individual service plans to ensure such plans are consistent with the person’s needs, preferences, and personal goals to make them to live, work, socialize, and participate in the community

  • Tracks individual staff action plans to ensure they are completed in a timely fashion; Assures that all plans address individual participants overnight needs/safeguards consistent with the Life Plan

  • Reviews monthly progress notes for participants to ensure that they reflect the person’s response to services provided and to assure compliance with OPWDD funding requirements; Tracks to ensure all progress notes are done contemporaneously to service provision

Our Requirements:

  • Master’s Degree in Rehabilitation Counseling, Psychology, Social Work or closely related human services field PLUS one-year full time paid experience with developmentally disabled individuals

OR

  • Bachelor’s Degree in Rehabilitation Counseling, Psychology, Social Work or closely related human services field PLUS three years full time paid experience with developmentally disabled individuals

  • Have and maintain a valid New York State Driver’s License and an approved driving record

Coordinator of Day Habilitation

Position Summary:

Develops, coordinates, and supervises the service activities in Group Day Habilitation and approves all assessments, service plans and monthly notes, assures compliance with Agency standards and guidelines, performs related duties as assigned.

REPORTING RELATIONSHIP:

Receives administrative and program supervision from the Associate Director of Day Habilitation, provides direct administrative and program supervision to the Day Habilitation Managers.

Job Duties and Responsibilities:

  • Develops and implements an organized program of supervision for Day Habilitation Managers to meet mandated regulatory and funding requirements.

  • Monitors individual service plans to ensure such plans are consistent with the person’s needs, preferences, and personal goals to make them to live, work, socialize, and participate in the community; Tracks individual staff action plans to ensure they are completed in a timely fashion; Assures that all plans address individual participants overnight needs/safe guards consistent with the Life Plan.

  • Reviews monthly progress notes for participants to ensure that they reflect the person’s response to services provided and to assure compliance with OPWDD funding requirements; Tracks to ensure all progress notes are done contemporaneously to service provision.

  • Monitors and tracks individual Life Plans and Level of Care Eligibility (LCED) to ensure maximization of waiver funding for group day habilitation services. Monitors and assists Program Managers and Day Habilitation Managers with attendance procedures and other OPWDD requirements as they arise.

  • Facilitates Intake meeting with individuals and their families, advocate(s), care coordinator, and/or other service providers to develop a comprehensive Staff Action Plan.

  • Ensure consistent implementation of group day habilitation services and supports throughout site.

  • With Intake Department, manages the coordination of program tours for individuals, their families, and/or other service providers. With Associate Director, will meet and determine possible and appropriate placement.

  • Monitors implementation of service plans through records review, regular supervision and consultation; as needed participates in case conferences; assures regular reassessment of habilitation plans; assures plans and monthly notes are completed in appropriate time frames.

  • Implements programs to assist individuals to obtain the most integrated level of day services consistent with their desires and program plans.

  • Coordinates transportation for people utilizing the service including scheduling of transportation with contracted van company, distribution of bus passes, monitoring agency fleet, vehicle maintenance, processing of vehicle trip logs, gas cards and ensuring vehicle keys and materials are labelled and stored in a single location.

  • Determines need for equipment and supplies and makes recommendations to the Associate Director of Day Habilitation. Purchases supplies as needed for the program.

  • Recommends policy and procedure revisions, service agreements, and fee schedules to the Associate Director of Day Habilitation.

  • Ensures safety of individuals through regular monitoring of work assignments and environment.

  • Evaluate staff performance consistent with Agency performance evaluation guidelines and procedures; when appropriate recommends to the Associate Director of Day Habilitation promotions, increments, and terminations.

  • Selects and assigns staff within assigned program unit.

  • Convenes staff meetings for regular individual progress review, case load review, analyzing and resolving issues, disseminating information.

  • Recommends and/or coordinates staff training to ensure program quality and regulatory requirements are met.

  • Ensures maximization of service units through completion of weekly billing

  • Reviews & increases enrollment of program to meet revenue and annual budget goals.

  • Assists in the evaluation of program effectiveness from previously stated goals, objectives and criteria and makes recommendations to the Associate Director of Day Habilitation.

  • Assures that all statistical and service records are adequate, accurate and up-to-date, and that required reports are forwarded to the Associate Director of Day Habilitation.

  • Facilitates intradepartmental communication to promote effective provision of services for all program participants

  • Ensures cleanliness and organization of program spaces including storage spaces such as closets and cupboards.

  • Collects/tracks registration and monies from individuals for special events

  • Completes evacuation drills as required by OPWDD guidelines as well as maintaining/updating special needs evacuation list for the site.

  • Assists with internal/external audits of the program/site with guidance/support from Senior departmental staff.

  • Works with assigned Behavior Specialist to ensure BSP's and Informed Consents are up to date and accessible for staff use, tracking forms are filled out appropriately and filed

Qualifications and Education Requirements:

Bachelor’s Degree in Rehabilitation Counseling, Psychology, Social Work or closely related human services field PLUS three years full time paid experience with developmentally disabled individuals and at least one year of supervisory experience.

Have and maintain a valid New York State Driver’s License and an approved driving record. Ownership and access to an insured car in good operating condition is required.

The ability to communicate with individuals in a positive manner, demonstrate the ability to be a leader and serve as an appropriate role model to the people we support. Receive supervision instruction in a positive manner. Adhere to and administer Agency’s policies, procedures, rules and regulations. Maintain a professional attitude and appearance at all times.

  • Equipment Operation:

Good knowledge an ability to operate motor vehicles; the ability to use office equipment, such as, computer, copier, fax machine, etc. May come in contact with normal household dust/dirt.
