Mental Health Association of SCK OCK Care Manager in Wichita, Kansas

Overview: The Care Manager will work with the One Care Kansas (OCK) Team, Adult and Children’s Case Management, and Counseling Center staff to recruit, enroll and coordinate cases for OCK. The Care Manager is responsible for coordination of physical and behavioral health services and supports, with emphasis on linkages to community and social supports. The Care Manager will also aid in taking vitals and medication reconciliations for OCK clients. The Care Manager would complete Health Action Plans (HAPs) with consumers to identify health goals and develop appropriate action steps to meet those goals. The Care Manager could also assist with submitting medical forms for clients as well as answering phone calls in courteous, efficient manner for One Care Kansas (OCK) consumers.

Essential Responsibilities:

  • Recruits new OCK members to MHA from internal referrals and external resources.

  • Meets a minimum of once monthly with each consumer on case load. Client visits may happen in their home, at MHA offices, or in the community and would include completing Initial HAP and HAP reviews in a time manner.

  • Ensures that Initial HAP and HAP Reviews are completed at least every 75-90 days.

  • Sends the completed HAP, assessments, and medication reconciliation to all identified medical and behavioral health providers at the completion of initial HAP and at each review update.

  • Completes and submits the HAP through the portal to all necessary providers.

  • Conducts health promotion activities by educating the consumer about their condition to build self-management skills: motivating members to participate in self-management, wellness groups, and prevention activities.

  • Ensures a high level of quality service provided to clients.

  • Enters and validates required data in multiple systems (EHR, State portals, etc.) with emphasis on accuracy.

  • Maintains accurate and timely documentation of service provision. Completes progress notes in a manner that individualizes each note, reflecting appropriate interventions and progress towards goals.

  • Completes initial and ongoing training as required for participation in the OCK program. Ensures compliance to practices of the programs.

  • Identifies gasp in care and schedules client appointments with appropriate community resources and medical providers to promote wellness and prevention strategies to health management. Communicates with clinical staff in a variety of medical and behavioral settings to aid in coordinating care.

  • Operates as a team with co-workers by approaching relationships in a manner which avoids antagonism and reduces conflict.

  • Attends and participates in meetings and is responsible for all information communicated at meetings.

  • Actively participates in and encourages actions that promote good public relations with clients, families, and the community.

  • Ensures proper utilization of supplies and resources.

  • Assists with transitioning clients into and out of inpatient settings by coordination and direct support to comply with follow-up instructions.

  • Participates in and facilitates individual and group classes which promote physical wellness, behavioral health management strategies, and topics related to social determinants of health.

  • Assists clients in connecting with resources in the community such as housing, medical providers, job training, education, and others as need dictates.

  • Provides support to OCK consumers and their families to help them meet their health goals.

  • Uses personal vehicle to transport consumers to appointments, community resources, and other uses as needed.

  • Educates clients about appropriate use of emergency and urgent care services with the aim of reducing unnecessary and preventable Emergency Room Visits and hospitalizations.

Other Position Responsibilities:

  • Maintains acceptable overall attendance record, to include department staff meetings, agency meetings, and trainings as required. Ensures appropriate notification to supervisor for absences and ensures that work is covered. Flexible in work schedule when needed.

  • Exhibits appropriate level of technical knowledge for the position.

Position Requirements:

The Care Manager is expected to have a BA/BS degree or four years of equivalent education and/or experience working in the human services field. Computer literacy required. Preferred areas include knowledge of community resources, housing alternatives and vocational services; experience working in the community with individuals who have chronic illness such as serious and persistent mental illness, diabetes and asthma; ability to write and communicate verbally in a clear and concise fashion; and the ability to develop and maintain rapport with consumers, constituents and staff. A valid Kansas driver’s license and access to personal vehicle required. Must be proficient in Microsoft Office including Word, Excel, and Outlook. Experience with electronic health records is a plus. Ideal candidate will possess excellent time management and organizational skills.
