Compensation:

\$93,000 to \$104,000 annually. \$5,000 sign on bonus (50% paid at 90 days, and 50% paid at 180 days1). Monthly travel stipend.

Job Summary: Home Health Registered Nurses admit and administer skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Administrator and the Director of Nursing. Duties/Responsibilities: The essential functions of this position include--but are not limited to:

Evaluate patients referred to home health and determine the type(s) of services to be provided. Admitting, discharging, and recertification of patients on services. Interview the patient and family to determine appropriateness for services. Coordinate the development of patient care plans that specify the frequency and duration of the

service(s) provided. Complete all admission documentation in electronic medical record software (MatrixCare). Regularly reevaluate the patients care needs. Perform services in accordance with the plan of care.

Draw labs and administer injectable and IV medications. Prepare clinical and progress notes for each patient visit and summaries of care conferences on his/her

patients in a timely manner as per Agency policy. Coordinate services. Counsel the patient and family/significant others in meeting nursing and related needs. Participate in and present in-service programs. Process orders and notify physician(s) of patient needs and changes in condition. Supervise and teach other nursing personnel. Is available for on-call duty nights, weekends and holidays, as assigned. Marketing the agency in facilities, medical practices, specialty clinics and the community at large. Other duties as assigned.

Supervisory Responsibilities:

This position supervises LPNs. The employee should also be able to lift at least thirty (30) pounds. Expected Core Hours/Days of Work: Expected core hours of work are 8 a.m. to 5 p.m., Monday through Friday. Evening and weekend work may be required as job duties demand. Travel: This position regularly serves the agencys Wichita, Kan., and Emporia, Kan., service areas. Each service area is a minimum of a 30-mile radius from the locations office. Travel between both locations is required. Required Education and Experience:

Active Nursing License in the State o Kansas.

Successfully completed an accredited nursing program. Knowledge of medical terminology. Familiarity with technology to understand the basic functionality of computers, smart phones, and

navigation within common operating systems. Additional Eligibility Qualifications:

Successfully pass background and reference check requirement. Current, valid drivers license. Provide binders showing current automobile insurance meeting states minimum requirements.
