Westchester Jewish Community Services INTENSIVE CARE COORDINATOR in White Plains, New York

Overview:

Join our team as a Full-Time Intensive Care Coordinator. This position offers an exciting opportunity to be part of a dynamic program in Westchester County, collaborating closely with the Department of Social Services (DSS) at their office.

Compensation: $55,000 - $62,000/year

Responsibilities:

As a valued member of our team, your responsibilities will include, but are not limited to:

  • Providing direct care coordination to youth and their families, focusing on mental health needs.

  • Providing on-going consultation to the team, from a trauma and mental health perspective.

  • Developing and implementing plans to ensure the safety and well-being of at-risk youth, enabling them to remain safely within their homes and communities.

  • Working in the DSS Cross Systems Unit (CSU) to provide essential support and guidance.

    Requirements:

    We are seeking candidates who possess the following qualifications:

  • Bachelor’s degree in Social Work or related field

  • Experience working with youth and families

  • Possession of a car and a valid driver’s license

  • Bilingual Spanish/English proficiency preferred, though not required
