Position:Full-Time Bilingual Case ManagerLocation:Westbury, NYSchedule:Monday to Friday, 9:00 AM - 5:00 PMCompensation:$30 - $33 per hourReports To:Supervising Case Manager

Why This Role Matters:

As a Bilingual Case Manager, you will play a pivotal role in guiding individuals through the Family Court system, helping them connect with the treatment and support they need. Your ability to communicate effectively in Spanish will enable you to reach and assist a broader range of clients, ensuring they receive the care and resources necessary for their recovery.

Key Responsibilities:

  • Court Liaison:Act as a crucial link between clients and the Family Court System, facilitating smooth interactions and ensuring clients are placed in appropriate treatment programs.

  • Assessment & Referral:Conduct thorough screening and assessment interviews, arrange for placement in treatment programs, and provide trauma-informed case management.

  • Client Support:Maintain detailed client records, prepare and present progress reports to the Family Court, and investigate and arrange supportive services.

  • Transportation:Escort clients to treatment facilities when needed, using your own vehicle, and ensure timely and safe transportation.

  • Case Management:Manage a caseload of substance-abusing clients, complete evidence-based assessments, and make regular visits to treatment agencies.

  • Service Integration:Connect clients with internal EAC programs such as New Path, Health Home, CASA, and Supervised Visitation.

  • Testing & Monitoring:Conduct and document urine, breath, and hair follicle testing, and manage secure remote alcohol monitoring devices, submitting findings to the court.

What We’re Looking For:

  • Education:Bachelor’s degree in Social or Behavioral Science, Criminal Justice, or CASAC, with a minimum of two years of relevant experience.

  • Language Skills:Fluency in Spanish is required.

  • Experience:Proven experience in case management, with strong knowledge of substance abuse treatment and the Family Court System.

  • Communication:Excellent oral and written communication skills, with the ability to work collaboratively with treatment teams and court officials.

  • Driver’s License:Valid NYS Driver’s License and willingness to transport clients. Proof of valid NYS car insurance, Defensive Driving Certificate, and vehicle registration required within 30 days of hire.

  • Technical Skills:Proficiency in Microsoft Office, spreadsheets, email, and other relevant software.
