Great River Health Systems Social Worker M.S.W. | PRN | Palliative Care in West Burlington, Iowa
Minimum Hiring Wage:
From $26.89 per hour
Scheduled: Thursdays 8:00am - 5:00pm and when needed
Provides palliative care patients and their families with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services address their social, emotional, and financial needs. Services provided include clinical assessment, crisis intervention, brief therapy, case consultation with the healthcare team and collaboration with other professionals in the community. These services advise family caregivers, provide patient education and counseling, and make necessary referrals to community resources for continuity of care. Assists with completion of IPOST and advance care planninig documents and engages the palliative care patient and family in defining goals of care.
Bachelor’s degree in Social Work from a program accredited by the Council of Social Work Education or LBSW preferred
Bachelor’s degree in a Human Services-related field, ie. Psychology, Sociology, Criminal Justice, etc., required
Social Work experience in health care settings preferred
Social Work experience in other practice settings preferred
Maintains valid licensure/certification/registration
