District Health Department No. 2 Community Health Worker in West Branch, Michigan

Job Classification: Two (2) Full-Time Community Health Worker/Secretary AFSCME Position, Non-exempt, Grant Funded

Base Site: West Branch

Grade: 2

2024 Rate of Pay: $15.23/hr.

District Health Department No. 2 is a dedicated and passionate team that provides leadership in promoting environmental and personal health through health promotion, disease detection, disease prevention, education, and regulation. Under the supervision of the Community Health Director, serves as a secretary, with certification as a Community Health Worker (CHW), assigned to assist clients and their families navigate and link to community services/resources, with the goal of enhancing healthy outcomes. Works as part of a multidisciplinary healthcare team. Responsible for receiving calls and walk-in clients and providing them with information or referring them to the appropriate person or office. Performs a variety of clerical tasks in support of departmental programs. Travel is required.

· Screening, assessment, and referral of the target population to address Social Determinates of Health needs.

· Safeguard client privacy and all HIPAA regulations.

· Assist in identification of needs and connecting individuals and families to community services.

· Conduct home visits or attend community outreach events.

· Effectively manage a caseload and multitask workload.

· Competent and accurate data entry into computer software, such as (Microsoft Office, Outlook, EMR and similar products).

· Additionally, types, formats, edits, and proofreads a variety of documents and forms.

· Interacts with clients in a non-judgmental and professional manner, creating pathways to care and healthy outcomes.

· Utilize effective conflict resolution skills to serve clientele in a tactful, objective, and respectful manner.

· Answer telephones, schedules, screens, prioritizes, redirect calls, or take messages. Answers questions and handles

· routine matters.

· Greeting guests and clients as applicable and directing them to the appropriate location or person as needed.

· Ability to work effectively with clients, children and their families and linking them to community resources.

· Faxes, collects, organizes, files, prepares, and processes mailings, and electronically scans documents.

· Inputting and updating information in the agency's electronic records.

· Other duties as assigned.

· Possession of a high school diploma or equivalent. Associate degree and/or relevant experience is preferred.

· One (1) year of clerical experience preferably in a clinic or medical office or any equivalent combination of education, training and experience which provides the necessary knowledge, skills, and abilities.

· Experience in working with culturally diverse communities and families and the ability to be culturally sensitive and appropriate.

· Excellence in customer service demonstrated by good verbal communication skills with a pleasant telephone manner.

· Community Health Worker certification is preferred but not required.

· Valid Michigan driver's license; own means of transportation.

· Successful completion of Community Health Worker (CHW) Training.

· Initial and annual training pertaining to OSHA's Blood Borne Pathogen Standard.

A background check is required initially and may be conducted periodically for an individual hired, transferred, reclassified, promoted, or currently working in
