Healogics, Inc Wound and Ostomy Registered Nurse (RN) - PRN in Webster, Texas

Summary: The Wound and Ostomy Nurse (WON) provides, coordinates and supports clinical aspects of care for patients with complex wound, and ostomy care needs. Serves as a consultant, nurse educator, and provides recommendations for the management of patients with complicated fistulas, and ostomies, in addition to the prevention of pressure injuries and management of complex acute and chronic wounds. Scope: Impacts the quality of care for wound and ostomy care (WOC) patients across the inpatient and outpatient institution. Essential Duties and Responsibilities: • Manages outcomes for wound, and ostomy patients across the continuum of care by providing, coordinating, facilitating and supporting clinical aspects of evidenced based direct patient care. • Communicates appropriately to other employees and customers based on the situation and the age level of the person. • Acts as a specialty consultant to physicians, nurses, physical therapists and patients for provision of WOC for all areas. • Acts as patient advocate in delivery and coordination of patient care, and has great passion for the specialty of wound care, ostomy, and pressure injury prevention, as well as program development. • Preserves the integrity of the skin by implementing and monitoring skin preservation strategies. • Providing hands-on wound care for complex non-healing wounds. • Provides stoma site marking prior to surgery as requested by a physician (as appropriate per certification/competency). • Serves as primary patient educator for all patients undergoing ostomy surgery as well as resource person for patients who have had previous surgery. Education includes pre-operative and postoperative teachings. • Collaborates with multidisciplinary teams to address WOC patient needs for acute care, patient/family education • Assures continuity of care through appropriate coordination of services and education and discharge planning for outpatient wound care as appropriate. • Coaches and assists team members to ensure optimum WON patient care and efficient and appropriate use of resources. • Performs and documents physical, psychosocial, and emotional assessments for all assigned patients. • Integrates knowledge of age-specific care, principles of specialty practice, institutional goals and standards, current professional practice standards and WON Standards in providing care. • Initiates or maintains actions to assure continuity of safe and effective care for patients between shifts, units and upon discharge for outpatient care as applicable. • Evaluates implemented interventions and modifies care according to evaluation, recommendation, and per provider orders. Documents interventions and modifications according to plan of care. • Adheres to proper Facility and Department policies, procedures, clinical pathways and accepted best practice evidence based standards. · Serves as an educator in the treatment of patients with wounds, compromised skin integrity, fistulas, stomas for patients, patient families, nursing staff, physicians, administration and other disciplines to augment and support education services. Uses appropriate teaching-learning principles in order to best facilitate the information. Presents relevant in-services regarding the above mentioned topics as requested by education or nursing directors. · Participates in performance improvement activities. Completes education modules as assigned • Provides efficient and effective care, attends where scheduled and is highly dependable. Licensure/Certification o Required: § Current State of Texas Professional Nursing License (RN). § Basic Life Support (BLS) o *Required: Upon hire or within twelve (12) months of hire, the applicant requires both current wound, and ostomy specialty credentials from either: § Wound, Ostomy and Continence Nursing Certification Board (WOCNCB): - Certifications: Certified Wound and Ostomy Nurse (CWON) § National Alliance of Wound Care and Ostomy (NAWCO): - Certifications: Wound Care Certified Specialist (WCC), & Ostomy Management Specialist (OMS) § American Board of Wound Management (ABWM), & National Alliance of Wound Care and Ostomy (NAWCO): - Certifications: Certified Wound Specialist (CWS) via ABWM, & Ostomy Management Specialist (OMS) via NAWCO
