    Summary:

    Assist Individuals with a disability to lead a self-directed life and contribute to the community. Assist with activities of daily living if needed and encourage attitudes and behaviors that enhance community inclusion.

    Essential Job Functions:

  • Act as mandated reporter if there is reasonable suspicion to suspect abuse or neglect of Individuals and make all required reports in established timeframes, as required by the NYS Protection of People with Special Needs Act.

  • Maintain certifications to distribute medications to Individuals as assigned, perform Strategies for Crisis Intervention and Prevention (SCIP-R) and/or PROMOTE, and perform First Aid and CPR.

  • Must be able to enter the swimming pool to ensure safety while individuals are utilizing the pool as assigned.

  • Prepare meals for individuals while adhering to recommended diet/meal plans.

  • Perform and/or assist individuals with personal hygiene tasks as well as performing daily home tasks as needed.

  • Complete all documentation related to daily services provided within required guidelines.

  • Transport individuals in an Agency vehicle.

  • Maintain all required trainings (classroom and online) relevant to the position.

  • Comply with and adhere to established NADSP core competencies/goals.

  • Demonstrate respect for individuals, diversity, and inclusion.

  • Provide supports to individuals by assisting in the development and implementation of all support plans.

  • Assist individuals in achieving their personal goals and desires.

  • Provide interaction and choices for activities.

  • Support individuals to be independent in activities of daily living.

  • Partner with individuals to promote their health and safety.

  • Promote positive behavior and supports through partnership with individuals and families.

  • Assist individuals to overcome barriers and challenges to establishing and maintaining a network of relationships and community inclusion.

  • Support the individual with educational, employment, and career goal attainment.

  • Perform all other duties as assigned.

    Knowledge, Skills, and Abilities:

  • Ability to work independently and motivate others.

  • Ability to read and write English.

  • Ability to communicate effectively, both orally and in writing.

    Education and Experience:

  • High School Diploma or equivalency

    Physical Requirements/Working Conditions:

  • Must be able to lift 50 lbs dead weight

  • Must be able to push/pull up to 25 lbs. frequently

  • Must be able to stand, walk, and stoop frequently.

  • May occasionally kneel or crouch. Kneeling is defined as bending legs at knees to come to rest on knee(s). Must be able to kneel in emergency situations to administer CPR. Crouching is defined as bending body downward and forward by bending spine and legs.

    Corporate Qualifications/Expectations:

  • Must be at least 18 years of age

  • Must meet applicable Vehicle Operator Requirements. (NYS Driver’s License required)

  • Adhere to all CDS Life Transitions, Inc. policies and procedures.

  • Adhere to the Agency Mission, Vision, Shared Values, and Customer Service Standards.

  • Attend mandatory education and training modules as scheduled; obtain and maintain all required certifications/training by State regulations and CDS policy

  • Act as a professional representative of CDS Life Transitions, Inc. in regards to appearance, behavior, temperament, communication, language, and dress.

    The listed salary range represents the organization's good faith and reasonable estimate of the range of possible compensation at the time of posting. The offered salary will be determined by: Applicant qualifications and experience, education, position specific licensing/training and departmental budgets.
