Responsibilities:

• Participates in Treatment Planning and collaborates with PT in making referrals to other disciplines or agencies.

• Selects therapy activities which are within the guidelines of PT standards of practice, and are patient specific.

• Provides direct care to patients with ongoing interpretation of patient responses.

• Informs PT of changes in patient response and activity that may affect the established plan of care.

• Daily documentation includes skilled need and includes, but is not limited to, present level of function, current needs, patient/family goals, patient/family education and treatment plans.

• Keeps PT and the treatment team informed of progress and needed revisions to plan and documents.

• Recognizes, reports to PT any situation that warrants discontinuation of treatment.

• Keeps informed of the latest technological developments and maintains professional competence and licensure requirements through various continuing education programs, in-services, seminars, and by reading relevant materials

• Participates in chart audits.

• Assists and orients new personnel to the department and organization.

Schedule: Day shift; various start times including 7:30AM, 8AM, 9:30AM, and 10AM, but can vary depending on caseload needs. Rotating days, weekends and holidays.

• High school diploma or GED equivalent required.

• Associate Degree as a Physical Therapist Assistant from an accredited program required.

Licensure, Certifications, and Clearances:

• Physical Therapist Assistant License

• Act 33, 34, 73
