The Program Coordinator Quality, Clinical Documentation Improvement (CDI) and care retention will report to the Program Leader, Quality,CDI and care retention, BILHPN and be responsible for supporting all facets of design, development, implementation, evaluation and continuous improvement of quality and CDI programs. This work may include development and presentation of materials, monitoring performance data, standardizing and aligning efforts across performance teams, as well as other responsibilities. This role will collaborate closely with the BILHPN Performance Management team and be a content expert and resource to support program implementation and success.

Job Description:

Essential Duties & Responsibilities including but not limited to:

  • Support oversight of performance related to specified population health programs.

  • Contribute to strategy discussions related to current and future specified programmatic needs to support BILHPN goals.

  • Maintain subject matter expertise in relevant program domains as well as understanding of national trends/approaches/literature in the related space.

  • Support continuous monitoring of program results and changes as needed to achieve goals.

  • Support program development and implementation addressing the following domains: Patient activation, Closing gaps in care, performance monitoring, training and education, strategy, collaboration with key stakeholders.

  • Collaborate with BILHPN clinical leaders in program development.

  • Maintain strong relationships with key BILH/BILHPN stakeholders and understand concerns/challenges when developing and implementing programs. Influence stakeholders and leaders to embrace programs and help deliver outcomes.

  • Work closely with BILHPN Operations/Performance Management to facilitate program implementation.

  • Ensure relevant BILHPN local care unit (LCU) support teams are connected across specific roles and have appropriate data and tools to execute the specific program/initiative.

  • Leverage data to understand performance, opportunities and support program goals.

  • Collaborate with data analytics team to support development of optimal reporting, scorecards and visuals.

  • Collaborate with Epic Team to understand and develop point of care tools that may enhance LCU support teams’ and/or practice care teams’ ability to be successful in relevant BILHPN programs.

  • Develop and support a culture of collaboration, teamwork, accountability and continuous improvement.

  • Have up to date awareness and understanding of latest technology/vendors available to support program performance.

  • Have up to date understanding and knowledge of specific BILHPN contractual goals with both public and private payers as relates to specified programs.

  • Perform all other duties as needed or directed to meet the needs of the department.

Minimum Qualifications:

Education: Bachelor’s degree required.

Licensure, Certification & Registration:

Experience:

  • Experience in the pop health/value based care environment preferred.

  • Experience with use of clinical and claims data to support pop health programs preferred.

  • Program management experience preferred.

Skills, Knowledge & Abilities:

  • Strong organizational and time management skills.

  • Collaborative and team player.

  • Knowledge and understanding of value based care, risk contracts preferred.

  • Understanding of process improvement tools and techniques.

  • Facility with Microsoft Word, Excel, Power Point.

  • Experience with Epic, Epic Healthy Planet and Arcadia preferred.
