MAAC (Metropolitan Area Advisory Committee) Community Health Worker in Vista, California

Location: North Campus Administration - Vista, CA

Hourly: $24 - $25 

Full-Time Temporary: Up to Five (5) Months

Under supervision of the Community Engagement Manager the Community Health Worker will serve up to 5 months as a Community Health Worker at the North Coastal Public Health Center.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES

The Community Health Worker (CHW) plays a critical role in bridging the gap between the community and healthcare services. Their responsibilities include:

  • Outreach and Education: Engage with the community to provide education on various health topics, preventive care, and available resources. Ability to build trust and rapport with families, actively listen, and convey information clearly and empathetically. Strength in explaining complex health information in a way that is accessible and understandable to families with varying levels of education.     

  • Deliver health education to families in maternal child health home visiting programs, utilizing provided materials and training.

  • Advocacy and Support: Advocate for the health needs of community members, assisting them in navigating healthcare systems, and connecting them with appropriate services and resources.

  • Serve as a liaison between healthcare providers and the community, ensuring that services are culturally and linguistically appropriate.

  • Resource Connection: Assist individuals and families in accessing social services, including food assistance, housing support, and mental health resources.

  • Data Collection and Reporting: Gather information on community health trends, needs, and outcomes to inform program development and report to MAAC and the coalition.

  • Collaborative Partnerships: Work closely with healthcare providers, community organizations, and the San Diego Community Health Worker and Promotora Coalition to coordinate efforts and enhance service delivery.

  • Tailor education sessions to meet the cultural and linguistic needs of the community, ensuring comprehension and engagement.

  • Provide families with referrals to appropriate county and community resources, ensuring they receive the necessary support.

  • Guide families through the application processes for county services and community resources, offering step-by-step assistance.

  • Maintain up-to-date knowledge of local resources and services, including those related to maternal and child health.

  • Conduct outreach calls to connect with families, provide program information, and encourage participation in services.

  • Engage in community events and activities to raise awareness about maternal child home visiting programs.

  • Build and maintain relationships with community partners to enhance service delivery and outreach efforts.

    The CHW is instrumental in improving health outcomes by empowering communities, reducing barriers to care, and promoting health equity.

    QUALIFICATIONS AND SKILLS

    Knowledge of: 

  • Cultural Awareness: Understanding and respecting the cultural backgrounds of the communities served, with proficiency in English and Spanish.

  • Communication: Ability to build trust, listen actively, and clearly explain health information.

  • Community Engagement: Skilled in connecting with families, building relationships, and conducting outreach activities like home visits.

  • Maternal and Child Health Knowledge: Familiar with key topics like prenatal care, breastfeeding, and early childhood development.

    EDUCATION/EXPERIENCE/CERTIFICATION

  •     High School Diploma or GED

  •  BA in Public Health, Social Work, or similar education is highly preferred but not required.

  • Understanding of basic health concepts, particularly related to maternal and child health.

  • Experience in health education, public health, or a related field.

  • Prior experience conducting health promotion activities, workshops, or one-on-one counseling sessions.

  • In-depth knowledge of local community resources, county services, and support systems available to families.

  • Understanding of the application processes for various services, including health insurance, food assistance, and housing support.

  • Experience in case management, social work, or a related field that involves connecting individuals to resources.

  • Familiarity with the healthcare system, including scheduling processes, referrals, and care coordination.

  • Deep understanding of the cultural dynamics and values of the community being served.

  • Awareness of cultural competence principles and how to apply them in healthcare and social services settings.

  • Experience in roles that require cultural mediation, such as community health work, social work, or patient advocacy.

  • Understanding of data privacy and confidentiality regulations, particularly in healthcare and social services.

  • Experience in collecting, managing, and analyzing data, ideally in a healthcare or social services context.

  • Prior experience maintaining accurate records and producing reports based on collected data .

HEALTH STATUS

Must have a physical examination, TB clearance and a drug screen by a doctor/clinic designated by MAAC as well as immunization records for influenza*, pertussis and measles as required by state and federal regulations prior to the first day of employment.

Physical Demands

While performing the duties of this class, an employee is regularly required to stand, walk and sit, talk or hear both in person and by telephone, uses hands to finger, handle or feel objects or controls, reach with hands and arms. An employee also is regularly required to stoop, kneel, bend, crouch or crawl, climb or balance and lift up to 50 pounds. Specific vision abilities required by this job include close vision, distance vision, depth perception, color vision and the ability to adjust focus.

Mental Demands

While performing the duties of this class, the employee is regularly required to use written and oral communication skills; read and interpret information; analyze and solve problems; observe and interpret people and situations; use basic math; learn and apply new information or skills; work under intensive deadlines and interact with MAAC management, administrators, staff, teachers, parents, family members, children and others encountered in the course of work.

BACKGROUND CLEARANCE

All positions are subject to a background/education check, DMV check, NSOR, exclusion and debarment checks prior to hire and periodically throughout employment as required by various funding sources.

Note: California Start law requires background criminal checks on anyone who works in a licensed childcare facility who is not a client. Must have or obtain a current criminal background check clearance, signed criminal record statement, and signed Acknowledgement to Report Child Abuse form prior to the first day of employment.

TRANSPORTATION

Must have and maintain a valid California Driver’s License, daily use of a personal, insured vehicle, and have and maintain a motor vehicle driving record acceptable to MAAC’s insurance carrier. Incumbents will be enrolled in the California DMV Pull Program.
