SUMMARY

Under general direction, the Senior Population Management Coordinator is responsible for monitoring and coordinating the day-to-day operations of a team of Population Management Coordinators and, if applicable assistants. Is responsible for population health, safety net, quality, and/or risk adjustment improvement activities at assigned practice locations or via a centralized population health related programs. Monitors performance metrics for the team and identifies and articulates opportunities for improved efficiency and adherence to department standards. Analyzes data to make recommendations to department leadership on workflow changes to improve quality of care related to the organizational strategic quality and safety goals. In addition to oversight tasks, serves as the organizer and driver of population management, quality, and risk performance at assigned clinics or specific clinical departments. Presents regularly to clinicians and site leadership on individual performance and recommends education and workflow changes for strategic improvement.

With support of their team members, manages patient outreach for discrete patient populations such as those with chronic illnesses (e.g., diabetes, hypertension and cardiovascular disease), patients needing preventative screening tests or patients that falling into the Safety Net program. Builds relationships with patients in order to assist the primary care team in developing an effective and accessible plan of care and ultimately tracks adherence to this plan of care.

The Senior Population Management Coordinator functions either as direct supervisor or functional lead and is involved in recruitment and directly oversees the onboarding, and training of the new Population Management Coordinators along with their day-to-day oversight.

Note: The Senior Population Management Coordinator involves complex and detail-oriented responsibilities, the ability to understand and articulate clinical terms and processes, and the ability to interact with patients and clinicians about preventative health, chronic disease management and accurate documentation practices

EDUCATION/LICENSES/CERTIFICATIONS

High School diploma or equivalency certificate (e.g. GED, HiSET, TASC Test) from an accredited institution or governmental unit required. Some college preferred.

EXPERIENCE

Skills and experience typically acquired through three years in a clinical business support, research, or ambulatory care setting. Previous training or experience work in population health management preferred. EMR experience and/or aptitude to master the EMR based on other technology experience required

  • Excellent time management and organizational skills required with the ability to manage multiple quality initiatives.

  • Ability to work independently as well as within the network for the clinical department and the larger population management coordinator group.

  • Ability to work and make decisions in a fast-paced environment.

  • Leadership potential.

  • Proficiency in data management, including the ability to extract data and develop spreadsheets, required.

  • Knowledge and understanding of medical terminology strongly preferred.

  • Excellent phone skills, strong customer service and ability to build relationships with patients by phone are required.

    * Proficiency in Microsoft Office, especially Excel is required
