Job Description
Job Title: Community Habilitation Worker or DCW
Reporting to : FI Supervisor
General Functions :
The Community Habilitation Worker or DCW is responsible to make sure that all people
with Intellectual and Developmental Disabilities (IDD) are effectively supported. The
intensity of the needed support will vary from person to person.
The Community Habilitation Worker or DCW is to ensure above all else the health and
safety of all individuals.
The Community Habilitation Worker or DCW is responsible to TEACH all activities of daily
living and self-care as well as communication, self-regulation and direction, and
socialization skills.
The Community Habilitation Worker or DCW will abide by the code of ethics of this
profession and is a skill builder first.
The Community Habilitation Worker or DCW will assist people to live in a manner that is
safe and beautiful.
The Community Habilitation Worker or DCW will make sure to get receipts whenever he or
she makes a purchase or assist a person to make a purchase.
The Community Habilitation Worker is to make sure that data is collected to allow for
assessment of progress or regress .
The Community Habilitation Worker will perform the fire drills in the assigned home at
least quarterly.
The Community Habilitation Worker or DCW will accompany people to scheduled medical
appointments as well as to the hospital as the need dictat es.
The Community Habilitation Worker or DCW will supervise the activities related to meal
preparation and ensure that meals are eaten safely and in the prescribed consistency both
at homes and in restaurants.
The Community Habilitation Worker or DCW is to provide recreational activities that are
developmentally appropriate.
The Community Habilitation Worker or DCW will ensure that the rights of people with IDD
are upheld at all times and take actions when those rights are not.
The Community Habilitation Worker or DCW will be trained to perform the medication
administration functions.
The Community Habilitation Worker or DCW will assist with food shopping.
Major Functions :
The Community Habilitation Worker or DCW will be familiar with governing regulations
so as to guide his or her activities in manner consistent with good practices.
The Community Habilitation Worker or DCW will ensure that weather appropriate
clothing are worn when being in the community.
The Community Habilitation Worker or DCW is responsible to keep a vehicle log
whenever he or she uses the agency vehicle.
The Community Habilitation Worker or DCW is to ensure that data is collected
accurately and contemporaneously.
The Community Habilitation Worker or DCW will work closely with his or her supervisor
to ensure that every person he or she supports have their wishes and wants met as
understood in the context of health and safety.

The Community Habilitation Worker or DCW will be sure to implement the habilitation
plan as instructed and written.
The Community Habilitation Worker or DCW is expected to process his or payroll in an
accurate and timely fashion using our current system.
The Community Habilitation Worker or DCW will ensure that pertinent information is
well documented and correctly relayed after they work their shift.
The Community Habilitation Worker or DCW will advise his or her supervisor of any
training needs he or she may have.
QUALIFICATIONS:
High School Diploma with at least six month of direct care experience OR
Bachelor degree in a human service related field
AND
Have general knowledge of governing regulations, be AMAP certified, SCIP-R train ed,
have a valid NYS license and passed IAHD’s driving test ABILITI ES
Ability to manage a culturally and educationally diverse work forc e
Excellent presentation and writing skills
Ability to translate agency philosophy into practice and implementable techniques and
interventions
Be able to represent the agency professionall y
Good command of a word processing and spreadsheet program
Bilingual ability highly desirable

PHYSICAL DEMANDS:

He/she must provide direct and situational intervention when an individual’s behavior
requires it, including crises prevention and physical crisis intervention (after
appropriate training has been given ).

He/she must be capable of assisting in the evacuation of individuals during fire drills
and other emergency situations.
He/she must be capable of assisting non - ambulatory individuals in transferring from
wheelchairs or other situations. Ex. for toileting .
All staff is considered generalists and is expected to do any job assigned in giving
service to an individual.
SPECIAL NOTE
All staff may be deployed without notice based on the needs of the residential
•
program, adult day services and or transportation. Seniority will be taken in
consideration as appropriat e. Overtime may be mandated based on seniority. Staff
transfers are at the discretion of agency administration.
HOURS AND TIME OF WORK
The Community Habilitation Worker or DCW is contracted for 40 hours a week and is scheduled
to work as the individuals needs dictate and as the agency representative sees fit.
