Responsibilities The VISN 5 Geriatrics and Extended Care Program Manager responsibilities include geriatric evaluation and management; Institutional Care: Community Living Centers (CLC), Inpatient Hospice, VA, state and contract nursing home care; VA and state domiciliary care; Non Institutional Care: Purchased skilled and non-skilled care, Home Based Primary Care (HBPC), home hospice (HH), homemaker/home health aide, home hospice, respite care; VA and community adult day health care, medical foster home care, Veteran Directed Care (VDC), and expansion of community based programs. The incumbent must have outstanding skills in planning, organizing, and directing programs within the VISN. Incumbent must be clinically knowledgeable of CLC Strategic Analytics for Improvement and Learning (SAIL) quality indicators. In addition, the incumbent must be fully and specifically knowledgeable of the VISN's operations. Incumbent must be cognizant of all active issues in the VISN. The incumbent is expected to organize, develop, and support the HPC/GEC program areas. The VISN 5 GEC Program Manager ensures collaboration and communication among the various program areas, respectively, in VISN 5 that will lead to measurable improvement in palliative care, geriatrics, and GRECC across the VISN. Develops and implements Quality Improvement plan for the VISN annually and reports plan and progress to VISN leadership quarterly. Reviews, analyzes and reports quality improvement data from the VISN 5 Bereaved Family Survey and other external survey tools related to patient satisfaction improvement. Develops a VISN 5 dashboard, prepares VISN reports and provide monthly/quarterly updates to VISN 5 Executive Leadership Council (ELC), VISN 5 Healthcare Delivery Committee (HDC), VISN 5 Rehabilitation and Extended Care Integrated Clinical Community (REC ICC) Subcommittee and associated communities of practice, VHA National Hospice and Palliative Care Program, and GRECC. Plans and administers VISN program virtual and face-to-face meetings to ensure ongoing development, education, training, and performance improvement initiatives. Provides technical assistance, policy interpretations and consultation on program and program evaluation/accountability issues through guidance materials, training workshops, on-site visits and verbal or written communication to the field. The position provides innovative leadership for a cultural transformation in VISN 5 Community Living Centers, and the rebalancing of resources, shifting from Institutional to Non-Institutional care, ensuring that the Network's mission is met while operating in accordance with the Network's policies, vision, and values, Veterans Health Administration (VHA) policies, and standards of Federal, professional and accrediting agencies external to VHA. Incumbent will perform the duties of this position in addition to other responsibilities as developed or agreed upon in concert with the Chief Medical Officer (CMO), Deputy Chief Medical Officer and the Network Director. Qualifications Basic Requirements: English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. (Reference VA Handbook 5005, Appendix G6); OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD). Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis.) NOTE: Grandfathering Provision - All persons currently employed in VHA in 0610 series and performing the duties as described in the qualification standard on the effective date of the standard (1/29/2024) are considered to have met all qualification requirements for the grade held including positive education and licensure/certification. The following Scope, Education and Dimension criteria must be met to qualify for Nurse IV: Scope: Executes leadership that is characterized by substantial and continuous responsibility and accountability for population groups or integrated programs that cross service and/or discipline lines and influence organizational mission and health care. Education: Master's degree in Nursing (MSN) and 4 years of professional nursing experience, one of which is specialized nursing experience equivalent to Nurse III and meets all dimension requirements for Nurse IV; OR Master's degree in a *related field with a BSN and 4 years of professional nursing experience, one of which is specialized nursing experience equivalent to Nurse III and meets all dimension requirements for Nurse IV; OR a Doctoral degree in nursing and 3 years of professional nursing experience, one of which is specialized nursing experience equivalent to Nurse III and meets all dimension requirements for Nurse IV; OR a Doctoral degree in a *related field with a BSN and 3 years of professional nursing experience, one of which is specialized nursing experience equivalent to Nurse III and meets all dimension requirements for Nurse IV. *Note: Foreign education programs/degrees are not creditable as related degrees. Preferred Experience: 1. Minimum 10 years-experience with geriatric extended care programming. 2. Experience with GEC program surveys (survey preparation preferred). 3. Minimum 5 years leadership or program management experience. 4. Experience working with multiple geriatric extended care programs, institutional and non-institutional. 5. Experience collaborating across multiple facilities and national program offices. 6. Experience with data analysis and developing performance improvement initiatives. 7. Experience with developing program goals and monitoring outcomes. 8. Knowledgeable of Palliative and Dementia care programming; Bereaved family survey (BFS) and other Veteran experience tools/monitors; CLC SAIL indicators and STAR ratings; Home Based Primary Care (HBPC) program standards (TJC survey experience preferred). 9.
