ABOUT US:

Volunteers of America Southwest’s (VOASW) Healthy Start (HS) program seeks to improve the well-being of mothers who are expecting or have just given birth and their partners through the delivery of individualized case management and care coordination. The HS program assesses the need of all individuals and families enrolled in the program and connects them with services and resources most appropriate for them. Through the delivery of these services, we hope every member can improve their quality of life, establish a support system and develop healthy living choices. The Health Coordinator (NP) is the primary point of contact for Healthy Start enrollees and directly provides care management and care coordination services. Health Coordinators are responsible for completing comprehensive assessments, establishing care plans, coordinating services and referrals and providing education and parenting support to help improve pregnancy and birth outcomes.

CLASSIFICATION SUMMARY:

Under the direct supervision of the Program Director , provides nursing care, prenatal/preventative health services (on-site, in-home, telehealth), oversees direct treatment, performs screenings and assessments, provides health promotion and parenting education, and supervises and evaluates general health of Healthy Start (HS) population.

ESSENTIAL DUTIES AND RESPONSIBILITES:

  • Gain client history and carry out assessment via observation, interview, and examination.

  • Select and order appropriate diagnostic and health maintenance procedures, order, perform, and interpret diagnostic studies, and plan for appropriate follow-up care.

  • Expedite the referral of clients to community health facilities, agencies, and resources.

  • Mediate pharmacological interventions and deliver direct treatment via prescriptive authority or referral to other healthcare providers or community resources.

  • Carry out preventative health assessments, screening, immunizations, and client care.

  • Act as a client advocate, defend clients' rights, and counsel and educate clients and their families/caregivers about preventative health, treatment options, and community resources.

  • Report on health care delivery, nursing processes and assist with health statistics compilation.

  • Assess client/family/community health needs, participating in interview and case finding.

  • Perform initial interviews, assessment, and preparation of clients, perform necessary nursing procedures and expedite medical regimens recommended by the Medical Director.

  • Assist practitioners with client care and coordinate screening projects (both on and off site).

  • Provide health education and counseling, along with training and supervision to volunteers, paraprofessionals, and other nurses.

  • Develop, implement, and evaluate treatment plans on maternal health clients, providing physical examinations on mothers, infants and children in accordance with IAW protocols.

  • Attend Community Consortium Quarterly meetings.

  • Ensure privacy, confidentiality, and coordinate client flow during clinical sessions.

  • Work as the primary care OB/GYN health care provider in an outpatient setting.

  • Monitor and supervise the quality of clinical techniques utilized by care managers, parent educators and interns.

  • Facilitate community consortium meetings as calendared and document attendance.

  • Recruit members for the consortium to reflect the best possible partnerships for HS population.

  • Work with staff to ensure that the outcomes related to referrals and partner management.

  • Work with all identified partners to establish a working relationship for ECM-HS for referrals and placements.

  • Support Healthy Start participants appointed to consortium.

  • Attend/facilitate Community Consortium Quarterly meetings.

Qualifications

CREDENTIALS, ABILITIES AND EXPERIENCE:

  • Must be certified by the State of California with 1 year of experience in maternal health, pediatrics, OB-GYN, or behavioral health populations.

  • Master’s degree in nursing from an accredited RN and NP Program, with current RN License.

  • Current Nurse Practitioner Certificate, as well as maintain a current national certification from approved organizations (American Academy of Nurse Practitioners, etc.).

  • Have strong background in health care policy, clinical reasoning, advanced health assessment and advanced women’s healthcare, among others.

  • Must be compassionate, empathetic and able to manage stress due to the high workload.

  • Experience and knowledge to educate and counsel patients, as well as document and up-date records; must possess a clear and concise writing and verbal communication skills.

  • Familiarity with emergency procedures, clinical policies, and referral procedures.
