JOB TITLE: Transportation Specialist and special projects assistant

STATUS:Non-exempt, full time

Hourly Rate:$18.00 to $22.00

SUMMARY

In this innovative role and service provision, the Transportation Specialist provides information, assistance, and referral services to older adults 60+, persons with disabilities of any age, caregivers, professionals and others assisting individuals with specific emphasis on information, referral & assistance with local transportation options. By utilizing local transportation providers and partners, seeks new opportunities for services and provides excellence in customer service will deliver the best options for our clientele to suit their needs. The role, in partnership with the Department Director, additionally participates in the oversight and development of the Here2There.help project.

DUTIES AND RESPONSIBILITIES

1.0 Provides information, assistance, and referral (I&R/A) services to older adults 60+, persons with disabilities of any age, caregivers, professionals and others assisting individuals with guidance with specific emphasis on information, referral and linkages to local transportation options.

1.1 Uses problem solving and customer service skills to triage consumer and professional inquiries.

1.2 Uses social work skills and a person-centered, strength-based approach to provide telephone assessment and consultation services.

1.3 May provide I&R/A, options counseling, advocacy, application assistance, and screening for long term services and supports and benefit/entitlement programs and transportation options.

1.4 Provides appropriate information about and linkages to programs and services to support an individual’s independence, choice and self-determination with follow-up to the extent possible to ensure consumer received the service(s) chosen.

1.5 Communicates in a professional, non-judgmental and culturally appropriate manner.

1.6 Maintains departmental and contractual goals for client response times.

1.7 Uses computer software to track all calls and requests for services and support information, in an accurate, timely fashion.

1.8 Maintains up-to-date resource files, shares pertinent resources with team members, and updates in-house and statewide resource listings, as needed.

1.9 Maintains up-to-date professional knowledge of the geriatric and disability fields, community services, entitlement benefits, clinical skills and competencies through reading, workshops and conferences.

2.0 Quality Oversight of Here2There

2.1 Provide quarterly quality assurance of vendor data

2.2 In the absence of the Department Director provide monitoring of Here2There email and inquiries

2.3 Participation in quarterly Stakeholder meetings and other meetings as requested by department director

3.0 Participates as a member of the inter-agency team.

3.1 Participates in all TRAC staff meetings and training.

3.2 Participates in all Lifespan staff meetings and trainings, including cultural competency and corporate compliance.

3.3 Participates in Quality Assurance activities.

3.4 Accepts and seeks guidance and supervision from the Regional Director of Mobility Management.

4.0 Other duties as assigned by supervisor.

Requirements

QUALIFICATIONS

EDUCATION: AAS in Human Services or equivalent degree.

EXPERIENCE: Two years in human services and or customer service, with at least one-year experience in the geriatric or disability field.

PHYSICAL AND MENTAL REQUIREMENTS

· Majority of work is performed at a desk or at a personal computer station.

· Intensive telephone and computer usage.

· Verbal, written, telephone communication and typing skills are required.

· Strong written, computer web-based proficiency needed.

· Interface with all levels of personnel.

· Ability to maintain composure under strict deadlines and difficult situations.

· Ability to work in multi-cultural situations.

If working less than 37.5 hours, accrued time is pro-rated.

Salary/Hourly Rate

Multiple factors will be considered for salary/hourly determination including, years of relevant service, skills required, education, and internal equity.
