Home Office: Vienna, IL, Golconda, IL, Karnak, IL or Anna, IL (can choose site that is closest to home residence).
Area of Focus: Behavioral Health
Program(s): 830 - Supervised Residential: The Supervised Residential Program provides adults living with substantial levels of mental illness or disability with 24/7 services through recovery-oriented community residential facilities.
Immediate Supervisor: Sr. Program Director
Supervises: Residential Technicians
Special Requirements: Participation in after-hours Crisis Call Rotation is required; evening/weekend availability is required; travel to off-site or community based locations is required; must be able to lift a minimum of 50 Lbs. unassisted. Should be able to lift 195 Lbs. with peer assistance or mechanical (hoyer lift). Must be 21 years or older to drive agency vehicles; travel to all agency sites when appropriate; maintain first aid and CPR certification; valid driver\'s license/minimum liability automotive insurance.
Skills & Knowledge: Must have knowledge of case management processes, area resources/service systems, crisis intervention/conflict resolution, word processing and basic computer skills. Employee must possess knowledge of community resource and human service systems, with the ability to represent the organization externally across a wide range of stakeholders. Excellent written and oral communications requires general knowledge of the regulations governing agency service programs, problem solving strategies, and supervisory skills are required.
Job duties include, but are not limited to:
Intervenes and evaluates clients, establishes diagnosis, as needed. Assists in the development of the individual treatment plan collaboratively with clients and the QMHP by defining goals, specific outcomes and time frames for treatment.
Monitor client\'s behavior.
Conducts crisis intervention to stabilize client\'s condition by utilizing short term crisis intervention techniques and/or referral to other support services.
Facilitates comprehensive consumer care by providing case management services and teach necessary life skills to residents of the facility.
Coordinates transportation services to assist clients in connecting with internal and external resources.
Respond to all requests for engagement at the SOPH within one business day of request by SOPH staff.
Assists staff in developing structure for regular occurring recreation/socialization and cultural activities for consumers that provide both skill building and an opportunity for community integration that encourages and prepares them to engage in activities independent of the agency.
Completes required monthly direct service hours to clients.
Provides clinical, administrative and program supervision/coverage to Residential Technicians - on site.
Monitors the achievement of service requirements amongst billing staff and participates in the issuance of disciplinary actions as necessary.
Conducts, reviews, and ensures the timely completion of probationary or annual performance evaluations; participates in employee disciplinary processes for staff under supervision.
Provides monthly individual and/or group supervision to all direct reports, under the direction of a QMHP; maintains a team-based environment to motivate and inspire staff to work collaboratively towards the Agencys mission/vision.
Ensures direct report staffs are facilitating linkage and referral to appropriate resources and opportunities as identified in the clients treatment plan.
