  • Behavioral Health: Outpatient Mental Health Programming helps individuals build life skills and well-being through behavioral health counseling, psychiatric care, and other care coordination and support services.
  • SAMHSA CCBHC serves individuals living with a mental or substance use disorder who seeks care, including those with severe mental illness (SMI), substance use disorder (SUD) including opioid use, children and adolescents with serious emotional disturbance (SED), individuals with co-occurring mental and substance disorders (COD), and individuals experiencing a mental health or substance-related crisis.

Programs Coordinator Skills & Knowledge:Counseling theory and practice, diagnosis and assessment, of SED, SMI, and COD; case management processes, area resources/service systems, written and oral communications, crisis intervention/conflict resolution, knowledge of family therapy, human development, ability to relate to children/adults, knowledge of foster care placements and knowledge of Juvenile Justice System, DCFS and Medicaid Administrative Rule 132/140, word processing and basic computer skills. Ability to monitor project outcomes and adherence to rules and regulations of program. Must possess administrative, supervisory, problem solving, decision-making, and analytical skills and demonstrate excellent communication skills, both written and oral, with the ability to represent the organization externally across a wide range of stakeholders.

Programs Coordinator Special Requirements:Travels to off-site/community-based locations for direct service delivery; participates in after-hours crisis response rotation; travel to all agency sites when appropriate; maintains and provides proof of a valid driver\'s license; maintains and provides proof of personal vehicle liability insurance; at least 21 years of age; operates agency vehicles and personal vehicles while driving for agency business in a safe and lawful manner at all times; maintain First Aid & CPR certification; schedules evening / or weekend hours as necessary.

Programs Coordinator job duties include, but are not limited to:

  • Coordinates all activities for the Golden Companions program, including: planning, outreach, education, monitoring disparity outcomes, coordination of volunteers and program participants, and promotion for seniors within the Arrowleaf service region.
  • Manage day-to-day operations of the Community Support Team program, including supervisory oversight, development of policy and procedure, and providing administrative support to direct service staff
  • Oversee staff performance and identify training gaps when needed to expend training funds available to the Community Support Team and Community Support Group staff
  • Assist with reporting outcomes for the Tobacco Disparity Grant in collaboration with CSG, 830, and the Director of Project Outcomes
  • Complete reporting to the Illinois Department of Human and Family Services for maintaining Arrowleafs license to provide CST and CSG services, grant reporting for CST, and other reporting for CCBHC as assigned
  • Lead and/or provide assistance with CCBHC committees to support project goals and objectives, specifically with the Client Retention and Engagement Committee and the Staff Training and Development Committee
  • Assist with community programming, specifically related to the Community Support Team and Group services, including assistance with arranging transportation and providing back-up coverage as needed
  • Collect National Outcome Measures (NOMs) data for three counties in collaboration with CCBHC leadership
  • Arran e and coordinate weekly CST meetings and group supervisions to ensure effective communication and consistent collaboration among program staff
  • Monitor client charts to ensure clients are up-to-date on required documentation, including completion of the Quality Assurance Utilization Report (QA/UR) reviews for the CST program
  • Conduct outreach activities to support the CCBHC project
