If providing hope and healing through compassionate, innovative, and individualized care interest you, we welcome your application for the position below.The Social Worker III (SWIII) performs advanced social service work within a social learning matrix that requires analytical skills, consistent clinical interpretation and sound judgment based on standard principles of Social Work. Serves patients with mental, emotional and behavioral conditions and addictions. Serves as a liaison between patients and their families in obtaining assistance and rehabilitative treatment, and identifies patient needs through psychosocial assessments; DRB Reports, treatment team collaboration, duties and meetings. Provides continuity of care functions and discharge planning with the patient, families, MHAand#146;s and other agencies, as appropriate. Demonstrates an appropriate level of clinical expertise in discussion of case dynamics, diagnostic practices utilizing DSM V and behavior of patient that contributes to the treatment teamand#146;s knowledge and understanding of the patient to provide the highest quality of care. Conducts psychotherapy for individuals and groups as determined by licensure and through recognition of special competencies. As designated by hospital policy serves as the Advance Directive Counselor to assist patients as needed in this area. The SW III reviews and finalizes social assessments for LBSW social workers (SW II). Supervises assigned social work student intern. The SW III works under minimal supervision with extensive latitude for the use of initiative and independent judgment; and functions under the supervision of the Chief Social Worker (SWIV) on the Program. If the SWIV is vacant, the SWIII will provide coverage functioning as the Lead Social Worker under the supervision of the Director of Social Work Services for clinical issues and the Program Director for administrative issues.This position may also be eligible to earn additional pay for work performed on maximum security locations.Essential Job FunctionsAttends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned. Supervises the completion of Social Assessments and DRB Reports as needed, prepares Social History Updates, and identifies educational needs of patient and family. Supervises assigned social workers and participates in the provision of discharge planning and continuity of care for patient through contacts with the patientand#146;s family (if consented by patient), mental health authorities, and other providers. Conducts individual and group sessions with assigned patients according to approved level of clinical expertise and the needs of the program. Supervise groups and treatment team coordinators.
