The Arc Erie County New York has been supporting children and adults with disabilities, and their families in achieving their desired quality of life by creating opportunities through comprehensive services for over 65 years.
Position Summary: Provides in-home and/or community-based training for family/caregiver authorized through the Children’s HCBS program and Erie County High Fidelity Wraparound. May provide services including but not limited to Family Peer Advocate, Parenting Skills training, Youth Skill Builder and Respite. Works closely with Children’s Mental Health community partners for opportunities to promote the expansion of children’s’ services. Maintains compliance with The Arc Erie County New York, Office of Child and Family Services (OCFS) and Health Care Integrator Agencies (HCIA).
Reporting Relationship:Receives general supervision from the Coordinator of Children’s Mental Health Services. Maintains regular contact with the Individual Care Coordinator/Case Manager from referring Mental Health Agencies.
Job Duties and Responsibilities:
Responsible to provide services as outlined in a referral, consistent with referral request and individualized care plan.
Responsible to develop/implement/maintain an individualized treatment plan consistent with family’s needs and service request.
Engage in teaching, training, and building upon already acquired skills and related to goals/strategies as outlined in the referral for services and Individualized Health Plan.
Responsible for detailed service documentation within the required timeframes.
Responsible to immediately notify the Mental Health Program Manager/Care Coordinator/Care Manager or on-call supervisor of the assigned Mental Health Agency (by following the emergency contact list) of any issues which might impact the safety of the identified person we support or the providers ability to perform his/her job duties.Must also notify the Coordinator of Children’s Mental Health Services.Must document all situations appropriately as defined by service regulations
Provide a therapeutic relationship one on one or within a group that focuses on the needs of the people we support and/or family.
Develop and maintain professional relationships with the family/caregiver and service providers.
Perform other duties as assigned, including program compliance, audit readiness and program expansion.
Follow compliance guidelines consistent with governing bodies and service regulations.
Qualifications and Education Requirements:
High school diploma, high school equivalency, or NYS Education Commencement Credential.
Must be the parent (foster, adoptive, or biological) or primary caregiver of a child or
adolescent with significant social, emotional, behavioral, mental health, medical, addiction,
or developmental needs with onset prior to the age of 21. (Child can be older than 21 at the
time of application).
Have or obtain and maintain certifications required, as soon as available, to include Evidence Based Parenting Skills TrainingANDNYS Credentialed Family Peer Advocate (FPA)ORProvisional Credential FPAORCertified Recovery Peer Advocate (CRPA) with a Family specialty.
A valid NYS driver’s license and approved driving record is required. Must have reliable transportation.
