Description Physical Therapist - SCS Home Health Schedule: Per Diem Day Shift Job Description: Physical Therapists develop, coordinate and maintain therapeutic services and activities alongside an interdisciplinary team to help patients in need successfully maintain or improve their level of independence. Required Qualifications: Bachelor's Degree in Physical Therapy from an institution approved by the State board. Upon hire: Washington Physical Therapist License (Vendor Managed) Within 30 days of hire: National Provider BLS - American Heart Association (Vendor Managed) Within 30 days of hire: National Provider Identifier (NPI) number Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request (SCS Home Health only). See policy for additional information. 2 years Therapy experience with adult physical disabilities.
