General Position Description:

The Program Coordinator will be responsible to the Associate Director and be responsible for the daily supervision of the Supported Living Program serving individuals with intellectual and developmental disabilities (IDD).

\ Nature And Scope:

\ The Supported Living Program operates year-round on a seven day a week schedule with individuals in community-based settings. The Program Coordinator will provide direct care, perform administrative duties, carry an employer-provided cell phone, and is required to work flexible hours.

\ Essential Duties and Responsibilities:

  • Responsible for hiring, disciplining, terminating, and overseeing training new staff. Maintain all required records and training documents.
  • Responsible for coordinating employee schedules and provide direct care.
  • Ensure all supervised staff have completed required trainings within the required time frames.
  • Monitor staff in the development of a variety of community activities and experiences.
  • Direct and monitor the work of Direct Support Professionals to ensure that agreements established in the Division of Developmental Disabilities (DDD) contract are followed and individual files are kept accurate and up to date. (Objectives, skill plans, data sheets, quarterly reports, professional contacts, progress notes, incident/seizure reports, face sheets, emergency cards etc.)
  • Provide opportunities for adults to assume and maintain valued roles. Show innovation and insight in developing the communitys ability to provide supports.
  • Supervise and train Direct Support Professionals on vehicle use. Ensure oil changes, routine maintenance and documentation is maintained and up to date. Submit documentation within established timeframes.
  • Maintain good working relationships with professionals, staff, families and person\'s served. Problem solve in a professional, non-hostile manner. Offer options and solutions and give feedback in a constructive manner.
  • Ensure monthly billing, attendance, time sheets and any additional paperwork is completed and distributed to appropriate administrative personnel.
  • Monitor member files for accuracy and ensure they are up to date with the correct information. Work with Direct Support Professionals and Supervisors to develop individualized support plans and training strategies, assisting them to communicate their needs and concerns, and promoting and monitoring the progress. Ensure all forms are kept up to date at members home.
  • Audit receipts for personal funds accounts and turn in to Administrative Assistant for reimbursement to accounts.
  • Approve staff payroll and time off requests in automated system. Ensure all supervised staff are completing required trainings within the required time frames.
  • Identify persons to serve in the Supported Living Program through home visits, evaluations and service providers.
  • Maintains client confidentiality and protects clients/staff by keeping information secure
  • Medication Management.
  • Coordinate regular staff meetings.
  • Attend team meetings.
  • Promote and follow practices that support health and safety of staff.
  • Carry an employer-provided cell phone; responsible for answering calls in a timely manner.
  • Conform to the policies, practices and procedures of EBF.
  • Complete continuous program quality improvemen t activities.
  • Other duties assigned by supervisor.
