Community Health Systems Patient Navigator (RN) in TUCSON, Arizona

Description

This is a full time, day shift Patient Navigator (RN) position supporting Northwest Medical Center.

The Patient Navigator serves as the liaison between the clinic, medical providers and the hospital center. The Patient Navigator will have primary responsibility for patient education, staff education in the clinic and evaluations for patients in the process of preparing for surgeries, procedures or other specialized medical care. In addition, the Patient Navigator is responsible for maintaining relevant policies and procedures, outcome data collection and quality improvement efforts.

Education:

Required: Associates degree or diploma in nursing program

Licenses/Certifications:

Required: Current AZ or compact state RN license
