Unity House Medical Transportation Coordinator in Troy, New York

Unity House of Troy, Inc., a non-profit multi services agency dedicated to enhancing the lives of people in need has an opening for a Medical Transportation Coordinator in our Supported Housing program.

Responsibilities:

Conduct program intakes.

Refer and link clients to transportation services.

Occasionally providing direct transport.

Coordinate services with other providers in the community.

Obtain, enter and track data for reports related to funding compliance related to medical outcomes.

Promote a culture of inclusion and belonging.

Requirements:

High School Diploma and 2 years experience working with adults and families living with HIV/AIDS. Experience working with adults living with mental illness a plus! You must have a Valid and clean New York State Driver License. Strong interpersonal skills and please be organized and ready to work independently and as a team player.
