Unity House Case Manager TAS in Troy, New York

Position is eligible for a $1250 sign-on bonus!

Transitional Apartment Services Program (TAS) is looking for the right individual to join the team as a case manager! Bring your positive energy to assist people on their journey in becoming independent!

Responsibilities:

Provide supportive services to program consumers with mental illness with compassion and dignity.

Maintain charts and documents within the guidelines of Office of Mental Health.Assist with daily living skills of the consumers.

Assist with light to moderate cleaning as needed.

Communicate with providers on a regular basis.

Promote an environment of inclusion and belonging.

Requirements: High School Diploma or equivalent and one-year qualifying experience. Experience working with multiple populations preferred. Must have reliable transportation and a valid NYS driver's license. Bi-Lingual candidates always welcome.
