Oakland Community Health Network Program Analyst - Certified Community Behavioral Health Clinics in Troy, Michigan

*Job Summary *

The Program Analyst implements OCHN's strategic vision and plan in support of the Certified Community Behavioral Health Clinics (CCBHC), including initiatives and projects that: 1) acts as primary point of contact and OCHN lead for CCBHCs, MDHHS, and other stakeholders2) expand service array and access to services for beneficiaries with mental health and/or substance use disorders; and 3) improve care coordination and integration efforts.   The CCBHC Program Analyst will lead OCHN's interests by facilitating local, state, and national collaborative projects to develop/refine policy and procedures that will further community inclusion, create efficiencies, and coordinate resources to ensure the best outcomes for those we serve.

Essential Functions

  • Leads the development, implementation, and management of Certified Community Behavioral Health Clinic (CCBHC) services.
  • Develops and maintains working relationships with primary and specialty care providers such as Federally Qualified Health Centers, Rural Health Clinics, inpatient hospitals, crisis service providers, SUD providers, MDHHS Integrated Healthcare Leads, CCBHC Leadership, and other PIHP CCBHC Leads.
  • Provides coordination of Certified Community Behavioral Health Clinics (CCBHC) activities   
  • Assists CCBHC with coordination and Outreach Efforts. 
  • Collects, analyzes, and reports on data that permits an evaluation of increased coordination of care and chronic disease management.
  • Monitors CCBHC performance and participates in quality improvement efforts.
  • Executes enrollments, disenrollments, and transfers using the MDHHS (Michigan Department of Health and Human Services) electronic enrollment system.

  • Conducts and Reports on audits as required by MDHHS to ensure enrolled individuals meet program eligibility.
  • Provides training and technical assistance. Engages in data management, reporting and population health management activities.
  • Manage/facilitate timelines and deliverables with internal teams to fulfill project requirements.
  • Participates in other OCHN integrated healthcare activities as assigned.

Administration and Collaboration

  • Provide population group content expertise and guidance to the Provider Network for the development and improvement of OCHN service initiatives, projects, and programs. This includes research, community input, management report analysis, etc. 
  • Provides information to OCHN workgroups and other internal and external stakeholders. 
  • Facilitates and participates in workgroups and committees with other OCHN Teams as assigned.
  • Advance and implement the principles and practices of Recovery, Self-Determination, Trauma-Informed Systems, Person/Family Centered Planning, and Best Practices, including the evaluation of outcomes and satisfaction surveys, as assigned.
  • Implement practices to address co-occurring disorders, including mental illness and substance abuse, as well as mental illness and developmental disabilities. 
  • Implement training to ensure that providers and staff have the necessary skills to support the goals of Certified Community Behavioral Health Clinics and advance OCHN's strategic plan. 
  • Implement identified initiatives and projects related to local, state, and federal collaborative activities.
  • Analyze data / management reports and provides recommendations about the need for Provider improvement plans as well as recognition for exemplary practices.
  • Performs other duties as assigned.
