Community Housing Network (CHN) is seeking a dedicated Supportive Services Coordinator (SSC) to provide housing-related support services to participants in our HUD-funded Permanent Supportive Housing Programs. The SSC serves as the primary point of contact for all active participants on their caseload, ensuring they receive comprehensive assistance in line with their Housing Case Plans. The SSC is responsible for supporting participants' progress towards identified goals and addressing both general and emergency issues that may arise. Beyond daily participant support, the SSC collaborates regularly with team members and external partners. In this role, interpersonal skills with the ability to build trusting relationships through collaboration will be vital. All duties will be executed within the framework of Housing First and trauma-informed care principles and the ideal candidate will prioritize cultural understanding, respect, resilience, adaptability, conflict resolution, and resourcefulness.

*Essential Functions:  *

  • Client Engagement
    • Conduct initial assessments to determine participants' housing needs.
    • Develop and implement individualized Housing Case Plans in collaboration with clients.
    • Provide ongoing support and guidance to participants in achieving their housing-related goals.

  • Case Management

    • Conduct monthly in-person Home Visit appointments.
    • Complete required documentation, including case notes, within established timeframes.
    • Ensure compliance with program policies, rules and regulations.
    • Collaborate with team members to address participants' needs and concerns.

       

  • Coordination

    • Work closely with other support agencies to address holistic participant needs.
    • Address and resolve minor housing issues and repair needs with landlords or property managers.
    • Collaborate with team members to ensure participant needs are met and program goals are achieved.

       

  • Data Management

    • Enter participant data accurately and timely into the Homeless Management Information System (HMIS).
    • Maintain electronic participant files and charts according to organizational workflows.
    • Assist with obtaining essential documentation to ensure compliant files.

Qualifications and Skills:

  • Minimum 2 years' experience with individuals who receive mental health services.
  • Demonstrated organizational, interpersonal, written communication, and oral communication skills.
  • Ability to work independently and collaboratively.
  • Valid driver's license and personal automobile coverage limits minimum: Bodily Injury $100,000 each person, $300,000 each occurrence Property Damage $100,000 each occurrence.
  • Experience working with people who are homeless is preferred.
  • Strongly desired experience with HMIS/Servicepoint.
  • Bachelor's degree in human services preferred. Equivalent experience is valued.

     

Travel:

This position requires frequent travel throughout Oakland, Macomb, and out-Wayne County. This travel includes meeting potential participants at their homes and occasionally in public places like libraries, coffee shops, fast food restaurants, parks, leasing offices, motels,
