Case Manager
The Rescue Mission of Trenton is seeking a Case Manager for our Emergency Shelter Unit. Under the direction of the Senior Case Manager, this position will provide direct case management and supportive services to emergency shelter clients with Substance Use Disorders.

This position is an important part of helping our clients navigate independent living.Qualifications For Position
Bachelor’s in Social Work, Counseling, Psychology, or related field (Master’s degree preferred).
Minimum of 3-5 years of experience in case management, social services, or substance use treatment, with at least 2 years in a supervisory or senior role
Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC)or Certified Alcohol and Drug Counselor (CADC) preferred
Valid Driver's License
Job Responsibilities and Duties
Conduct comprehensive assessments to evaluate clients' needs, strengths, and barriers.
Work with clients to develop individualized service plans with specific, measurable goals.
Identify potential referrals for treatment, housing, and other essential resources.
Coordinate access to substance use treatment, mental health services, medical care, and public benefits (e.g., Medicaid, SNAP)
Refer clients to programs, vocational training, or educational opportunities as appropriate
Facilitate communication with external service providers to ensure continuity of care.
Tracks client's progress toward service plan goals, adjusting plans as needed
Document client interactions and update case records
Address setbacks, relapse or missed appointments, identify solutions and coordinate care.
Coordinate emergency services, such as detoxification, hospitalization, or shelter relocation.
Work with clients to stabilize situations and revise service plans as needed.
Develop and implement discharge plans for housing, employment, and continued care needs
Ensure all referrals and follow-ups are confirmed before discharge.
Participate in case review meetings and interdisciplinary discussions.
