Case Manager
The Rescue Mission of Trenton is seeking aCase Manager for our Behavioral Health (BH) Unit. Under the direction of the Vocational Development Coordinator, the Behavioral Health Case Manager works with clients on income development, obtaining their identification and medical benefits and prepares them for employment and independent living upon discharge. We provide a safe, clean, warm refuge for the homeless, the hungry, the transient and the addicted. Our dedicated staff in the Shelter and our Behavioral Health Unit offer care, support and encouragement to all that reside here. This position is an important part of helping our clients navigate their independent living upon discharge.
Position Details
Meet with all clients to apply for all identification documents with specified timeframe
Meet with halfway clients to apply for social services benefits
Assist clients in obtaining SSI and SSD Income
Transport clients to and from established appointments
Collaborate with outside agencies
Follow-up with clients and external agencies for securing documents and benefits
Documentation of all updates on Master Roster
Entering case management notes in Credible System
Serve as Liaison for Vocational Team Roles
Qualifications:
Degree in social work, criminal justice or other social services field required
Two years experience in case management
Proficient in Microsoft Office Suite and Electronic Health Record
Well organized, able to prioritize among competing tasks and able to work independently
Fluent in English and Spanish preferred (both written and verbal)
Excellent relationship-building/people skills
Valid Driver License
Hourly Rate: $22.85 Monday - Friday 8a-4p, plus a comprehensive benefit and training package
