Reference #: 4623051 THE POSITION As a Blended Case Management County Caseworker, you will provide direct service to adults with chronic mental illness, emotional disorders, or substance abuse/addictions, and children with a serious mental illness or emotional disorder.

DESCRIPTION OF WORK As a caseworker in the Blended Case Management Program, you will ensure the clients eligibility and continued need for program services.  You will work closely with the clients mental health practitioners and provide consultation in crisis situations, as well as in the overall treatment and management of the clients mental disorder.

The Caseworker will participate in the on-call service.  Services will be provided in accordance with a written, client-specific service plan, which includes strengths as well as needs and is goal and outcome oriented.  Services will be delivered in accordance with Blended Case Management regulations dealing with mental health consumer rights.

Work Schedule and Additional Information:

Full-time employment. Work hours are 8:00 am to 4:00 pm, Monday - Friday, with a 30-minute lunch. Hours may vary due to travel, after hours appointments, etc. Telework:  You may have the opportunity to work from home (telework) part-time up to 18 days per year.  In order to telework, you must have a securely configured high-speed internet connection and work from an approved location inside Pennsylvania.  If you are unable to telework, you will have the option to report to the headquarters office in Towanda, PA. *  REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY*

QUALIFICATIONS Minimum Experience and Training Requirements: Six months of experience as a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or A bachelor's degree with a social welfare major; or A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social service agency; or Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences. Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.

Other Requirements: PA residency requirement is currently waived for this title. You must be able to perform essential job functions.

Legal Requirements: You must pass a background investigation. A conditional offer of employment will require a drug screening. This position falls under the provisions of the Child Protective Services Law. Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
