Obran Cooperative Occupational Therapist For Home Health Visits in Torrance, California

Physicians Choice Home Health is recruiting for an Occupational Therapist to join our Home Healthcare Team. In this role, you will be responsible for providing quality home health visits to our patients in the comfort of their home. You will be joining a team of compassionate and dedicated healthcare professionals who are committed to providing the highest level of care to our members.

Objectives:

  • Evaluates the patient's functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.). Evaluates home environment for hazards or barriers to more independent living. Identifies equipment needs. Participates in the development of the total plan of care.

  • For patients who plan to return to work, the occupational therapist may perform work capacity evaluation and may refer to specialized vocational training in accordance with Physicians Choice Home Health policy.

  • Develops a treatment program and establishes goals for improved function. Communicates plan of care to referring physicians and other Physicians Choice Home Health professionals.

  • May teach new skills or re-train patient in once familiar daily activities that have been lost due to illness or injury, in accordance with Physicians Choice Home Health policy.

  • May fabricate splints and instruct patient in the use of various types of adaptive equipment to improve function.

  • May train patient in the use of prosthetic and/or orthotic devices.

  • Maintains appropriate clinical records, clinical notes, and reports to the physician any changes in the patient’s condition. Submits these records in accordance with Physicians Choice Home Health policy.

  • Maintains contact/communication with other personnel involved in the patient’s care to promote coordinated, efficient care.

  • Attends and contributes to in services, case conferences, and other meetings as required by Physicians Choice Home Health policy to ensure coordinated and comprehensive plans of care for the patients of Physicians Choice Home Health.

  • Identifies patient and family/caregiver needs for other home health services and refers as appropriate.

  • Instructs the patient's family/caregiver and other Physicians Choice Home Health health care personnel in the patient's treatment regimen when indicated and appropriate.

  • Supervises certified occupational therapy assistants according to Physicians Choice Home Health policy and state regulations.

  • When therapy is the only skilled service, instructs, supervises and evaluates home health aide care every two (2) weeks.

  • Participates in discharge planning for patient.

Requirements

  • Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.

  • Registered with the National Registration Examination of the American Occupational Therapy Association.

  • Two years of clinical experience as an occupational therapist.

  • Demonstrates good verbal and written communication, and organizational skills.

  • Possesses and maintains current CPR Certification.

  • Ability to be able to travel to different patients' homes.

  • Discretion in dealing with sensitive and confidential information

  • Prior experience working in a home health setting is preferred, but not required.

Rates

Physician Choice Home Health currently pays OTs $60-110/visit and $40/hour for in-services, trainings, and orientations. Per visit mileage reimbursement is offered for W2 employees.

Salary Range Disclaimer

The rates listed represent the low and high end of the range for this position. Visit and reimbursement rates vary based on visit type and employment status (1099 or W2). Rates may be adjusted in the future, and employees will be notified of these changes.
