Ministry Support (Diakon Lutheran Social Ministries) Case Manager II in Topton, Pennsylvania
Category/Shift/Hours per 2 week pay period: Full-Time / Day Shift / 75 hours per pay period

General Responsibilities:
- Responsible for serving as a resource person and case manager to support families involved in the Permanency Services program.
- Establishes and maintains professional and supportive relationships with youth, resource families, and birth families, referring personnel, treatment providers, and others involved in the lives of children and youth.
- Meets regularly in the youth's home to provide skill development and on-going case management services to coordinate the treatment planning and services provided according to the Treatment Plan and clinical direction.
- Maintains regular contact with referring personnel and attends and testifies at court proceedings involving children and youth in care.
Childline & DHS FBI clearance results are required prior to beginning employment.
Education/Training: A bachelor's degree in social service or a related field of study normally required; A Master's Degree preferred.
Experience: A minimum of two (2) year's related experience normally required.
ADA: Ability to sit, stand, bend, stoop, reach, and lift items weighing up to 50 lbs.; visual and auditory skills.
