The Community Health Worker (CHW) is an essential member of the Diversity and Inclusion Team. CHWs are frontline team members who have a clear understanding of the communities we serve. This trusting relationship enables them to serve as a liaison between the patient/family and the multidisciplinary healthcare team. CHWs also build individual capacity by increasing knowledge and self-sufficiency through a range of activities such as outreach, informal counseling, social support and advocacy. CHWs are dedicated individuals who function along a healthcare continuum ranging from individual development to service delivery and promoting patient/family empowerment and social justice. They help link people to needed health care information and services.
The Office of Diversity and Inclusion is dedicated to improving healthcare quality, equity, safety, compliance and experience for the hospital and patients/families while practicing from a lens of cultural humility and anti-racism. The CHW would support this team s mission by providing the critical component of a patient/family touchpoint to ensure the hospital is meeting the needs of our community and providing a conduit for the patient/family voice.
Job Qualifications:
Excellent problem solving skills
Detail oriented with attention to follow-up
Effective interpersonal, oral and written communication skills
Working knowledge of MS Word & MS Outlook
Working knowledge of safety event reporting software
Ability to work independently
Bachelors in related field preferred
Strong understand of cultural needs related to the Orthodox Jewish community.
Job Requirements:
Helping individuals/families develop their capacity, understanding and access to resources, quality care and health information via telephone, email and face-to-face interactions
Comfortable rounding & visiting with patients and their families.
Strong understand of cultural needs related to the Orthodox Jewish community
Linking people to health care/hospital resources; help patients/families in utilizing hospital resources and overcoming barriers to obtaining needed information and services
Facilitating communication and patient/family empowerment in interactions with health care systems
Document activities, service, and results in an effective manner while strictly adhering to the policies and procedures in place
Helping health care team become culturally relevant and responsive to their service population
Advocating for patient needs; providing informal counseling, support and follow-up
Work collaboratively and effectively within a team
Establish positive, supportive relationships with patients/families and provide feedback
Effectively work with people (staff, clients, doctors, etc) from diverse backgrounds and building and maintaining positive working relationships
Motivate patients to be active, engaged participants in their health
Continuously expand knowledge and understanding of resources, services and programs provided; human relations and the procedures used in dealing with the public; operations, functions, policies and procedures associated with the department
