Providence Medical Group (PMG) is seeking an experienced nurse practitioner or physician assistant to support our a team of internal medicine physicians at PMG Scholls Medical Plaza. You’ll be part of a collaborative, value-based, and patient-centered medical home made up of physicians, advanced practice providers, behavior health provider, clinical pharmacist, case management, and an experienced clinical support team. A great opportunity for an experienced APP to see a broad range of complex and chronic medical conditions for adult and geriatric patients in a busy primary care clinic. Just minutes from Washington Square, this community clinic in Tigard is in a fantastic location in the West suburbs of Portland, Oregon.

• Full-time, 1.0 FTE

• Monday - Friday, daytime hours, 4-5 days/week schedule

• Non-paneled, team-based care with direct and indirect patient care (In Basket management)

• Ideal candidate will be comfortable seeing adult and geriatric patients and proficient with managing complex care coordination

• 2 years experience required

Providence Scholls Medical Plaza is a full-spectrum Primary Care facility located in the beautiful community of Tigard, Oregon. The clinic works with patients of all ages and features robust service lines covering Internal Medicine, Family Medicine, Pediatric, Urgent Care, Rehabilitation and more. It also has the distinction of being a Primary Palliative Care Certified Clinic as well as earning the Patient-Centered Primary Care Home Clinic Certification. With a dedicated team and patient-centered focus, the Scholls Medical Plaza offers quality care to all it serves.
