Community Counseling of Bristol County Homeless Access Case Manager in Taunton, Massachusetts

Description of Position We are currently seeking a Coordinated Entry Specialist for the New Bedford Continuum of Care. This position will assist in locating, assessing, and referring those in our community facing homeless to the appropriate services to work through their housing crisis. You will work with a small team of specialists to maintain a list of everyone in our area currently experiencing homelessness, and ensure services are in place to move people from homelessness to housed as quickly as possible. As a member of the Supportive Housing team, you will triage with a multitude of other agencies to divert those at risk of homelessness, and work with clients in the community as well as in their homes. Strongly prefer multilingual (ability to speak and read either Spanish, Portuguese, Haitian Creole, Cape Verdean Creole) candidates. Responsibilities Coordinate with client, local housing and homeless agencies as well as programs working to house and stabilize those in a housing crisis Refer qualifying clients to housing programs and services including Continuum of Care housing, Flexible Services, and other subsidized properties to establish housing and services needed to be successful Train agencies and community partners on the Coordinated Entry system and how to help those in a housing crisis Divert clients who are facing homelessness to resources including their own support networks to prevent homelessness Outreach to client and community partners to establish a By-Name list of everyone in the area currently experiencing homelessness to help coordinate efforts to move them and to make homelessness episode short and non-repeating. Requirements 3-5 years of experience working with homeless populations, and/or homeless programs Knowledge of available referral sources for providing services to clients, including available community support and social service resources Ability to drive throughout the county and at times transport clients required Able to effectively interact with clients, co-workers, and volunteers with diverse ethnic and cultural backgrounds and treat each individual with respect, dignity, and compassion Communicate well both orally and in written form, and follow detailed instructions Comfortable using Microsoft Office 365 tools Preferred Qualifications ? Bachelor?s degree in Social Work, Sociology or equivalent education plus experience ? Multilingual (able to speak and read Spanish, Portuguese, Haitian Creole, Cape Verdean Creole)
