DaVita Medical Office Assistant / Care Coordinator - HYBRID - Washington in Tacoma, Washington

It takes a Village to care for our patients and as an IKC Medical Office Assistant (Care Coordinator/CC), you will be at the heart of all the action. Our CCs work together as a team to facilitate the planning and organizing of all critical activities for our patients. The patients we care for are some of the most complex patients and need an excellent care team to help manage their chronic conditions. The Care Coordinator works closely with our Nurse Practitioners, RN Case Managers to make sure our patients receive the highest quality care. Position Details: HYBRID schedule, working in DaVita clinics 4 days per week and remotely the remaining days. Full Time hours, Monday through Friday Starting time between 7:00 and 8:00, with flexibility when needed Commuting to multiple DaVita clinics in the Lakewood, Tacoma and Parkland WA market Mileage Reimbursement Opportunity to have Direct Patient Interaction and Build Relationships Professional Growth Opportunities / Tuition Reimbursement Options Diverse, community-based and FUN Work Culture Fortune World's Most Admired Companies Responsibilities: Assist the clinical team with moving the patient through the continuum of care by communicating with and completing tasks as directed. Assist the care management staff with implementation and documentation of the discharge care plan that includes initiation of referral forms, gathering clinical information, initiating the process of completing a chart and appointment scheduling. Communicate directly with patients on a timely basis for all scheduling requirements. Maintain accurate and complete documentation of records. Forms are fully-completed, legible, have no unapproved abbreviations and have the proper signatures. Ensure that documents get to appropriate parties in a timely manner, without compromising confidentiality or patient privacy procedures. Demonstrate exceptional two-way communication using clear and well-organized oral and written techniques. Act in accordance with the DaVita, Inc. mission and values to deliver high quality care to our patients. Qualifications of the Medical Office Assistant: High school diploma or GED required Experience in a medical office or hospital setting a bonus At least two (2) years of experience in a professional office environment Prior remote work experience is a bonus Ability to work with and resolve confidential information and/or issues using discretion and judgment Professional verbal communication and telephone etiquette Proficiency with Microsoft Office/Excel/Outlook Patient engagement - PATIENT-FOCUSED role!
