Shift:

Sunday- Thursday, or Tuesday- Saturday, currently 7-3:30pm

The Emergency Shelter Case Manager assists clients by assessing their needs and collaborating with appropriate services to aide clients in moving toward healthy independence. This position will assist in maintaining a structured, safe, and supportive environment which reflects the mission, vision and values of the Client.

Duties & Responsibilities:

-Carries caseload and maintains timely and accurate documentation.

-Identifies strengths and barriers for each client and completes case management assessments in a timely manner.

-Establishes and maintains rapport with each client. Providing supportive counseling, encouraging and motivating clients toward positive growth.

-Assists clients with identifying attainable goals and works with them to attain those goals.

-Monitors progress toward desired outcomes through assessment and maintaining consistent contact with each client.

-Reviews, modifies and follows up on client assessments and action plans within 3 days of receiving file.

-Refers clients to appropriate internal and external service care providers.

Networks with applicable individual, agencies, and organizations within the community. All partner agency communication (call backs or email response) must be responded to within 3 days of receiving call/email from provider.

-Maintains client professional boundaries with clients as well as confidentiality by keeping a strict "need to know" policy and monitoring the handling of confidential information.

-Collaborates with community services involved with each client; financial, mental health, addiction, legal, etc.

-Attends required meetings and events

-Promotes a safety-first attitude by setting a positive example and ensuring all employees are following safety policies and procedures.

-Regular and predictable in-person attendance is an essential requirement of the job

-Perform other duties as assigned.

Education Requirements:

-Associates Degree in human services or related field or equivalent work experience

-Bachelor’s Degree in social work or related field preferred

-Previous related work experience

Knowledge, Skills, & Abilities Needed to Perform Effectively in this Position:

-Ability to demonstrate sensitivity and compassion toward a vulnerable population.

-Ability to maintain a calm professional presence at all times while handling daily stress from working with individuals with complex issues

-Strong Communicate skills, both written and oral

-Strong interviewing and assessment skills

-Ability to use critical thinking and problem solving skills

-Strong Time Management

-Strong Computer Skills including Microsoft Office

-Team player

-Passion for the mission and vision of the organization.

Pay Details: $43,680.00 to $49,920.00 per year
