This position is responsible for assisting the Public Health Nurse Manager in organizing, directing, and evaluating the Public Health services provided by the Public Health Department. Activities may focus on the individual, the community, or the systems level and emphasizes health promotion, prevention of health problems, and harm reduction strategies.   The position uses sound professional judgment with significant oversight, conforms to adopted policies and procedures, solves problems and achieves results consistent with stated goals and objectives.

This position is given discretion, within the scope of policy and regulations, in the routine performance of their duties; supervision and guidance are received from the Community Health Director, Public Health Nurse Manager, and/or the Health Officer. 

[E]{.underline}[SSENTIAL FUNCTIONS: ]{.underline}Works within the public health team to accomplish local, state, and federal public health goals and policies.  Performs a variety of professional nursing duties in a generalized community health nursing program and/or specialty area including immunization, sexual and reproductive health, communicable disease, maternal and child health programs, Women, Infant, and Children (WIC), sexually transmitted disease, and/or family planning. Provides direct nursing services including assessment, immunization, investigations, and evaluation activities with significant oversight.    Provides options counseling, risk reduction and anticipatory guidance on a variety of health promotion topics, including reducing the risk of STD's and HIV, benefits of planned pregnancies and preconception counseling.  Investigates reports of communicable diseases, contacting persons and groups with potential exposure to the disease about transmission, treatment and prevention. Prepares written reports of cases investigated. Informs clients of other important health topics such as osteoporosis, heart disease, immunization recommendations as indicated by the client's history or physical findings. Coordinates appropriate referrals and follow-up of abnormal findings beyond the scope of advanced practice nursing or the scope of the public health clinic's responsibilities. Maintains proper, timely, and accurate documentation of all client encounters. Provides direct service to clients in a primary and back-up role.  Applies nursing process and Public Health knowledge in response to individual health issues.  Provides professional advice, consultation and services to other departments and general public.  Reviews County data for socio-demographic changes, rates of disease trends, nutritional needs, violence, accidental injuries, suicides, drug and alcohol arrests, health outcomes, morbidity and mortality, poverty rates, geographic trends within the County, immunization rates and infant death reviews. Assists in the formulation and promotion of health and social policies that affect the health of the community. Reviews program evaluation of structure, process and outcome of health care that may include:  cost benefit analysis, recording system, quality of intervention, immediate and long-term outcomes both intended and unintended. Collaborates with others including health care providers, community leaders and organizations to implement plans.
