Position Status: Full Time

FLSA Status: Non-Exempt

Community Health Action of Staten Island (CHASI)drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community.

POSITION SUMMARY:

CHASI’s Next Step Resource and Recovery Center hosts several services including peer driven individual and group recovery services, counseling and case management, overdose prevention services, harm reduction, a recovery-oriented community outreach, education, and the Arise Training Institute. The center is open 24 hours a day/ 7 days a week and provides opportunities for people in all stages and forms of recovery to deliver an attend individual and group support activities, initiate and conduct outreach and events celebrating recovery and reducing stigma associated with substance use.

The Counselor provides quality assurance, support, and guidance to ensure the highest quality services and documentation for Next Step resource and Recovery Center. As part of a multi-disciplinary team, the Counselor provides individual counseling, group counseling, support, and case management services. The counseling clinician assesses the client’s level of care and provides appropriate counseling, support, and referrals to improve overall wellness in areas including but not limited to substance use and mental health, medical health, and case management. The Counselor completes documentation with accuracy and compliance, provides counseling and supportive services to clients and members, facilitates skill building groups in addition to maintaining a caseload for recovery support, family counseling, and referral management.

NOTE: This position’s schedule is Sunday-Thursday, 12 pm -8 pm, unless otherwise specified.

DUTIES & RESPONSIBILITIES:

  • Complete diagnostic assessments, evaluation, intervention, and referral for individuals who self-refer, and/ or are referred by H.O.P.E/ Relay Programs for level of care and treatment options available to them. Work within the framework of client center treatment and support. Collaborate on the development of an initial participant action plan, including a thorough review of substance use and mental health related issues.

  • Complete biopsychosocial intakes, treatment planning, and discharge planning with continuity of care.

  • Coordinate and facilitate individual and family education that is responsive to specific interests and needs of the individual and or family which may include a focus on treatment, harm reduction, recovery support and relapse prevention.

  • Provide crisis intervention and support to program participants.

  • Engage in providing syringe exchange and harm reduction and prevention techniques and education. Provide Naloxone training to participants.

  • Maintain appropriate positive, professional relationships with staff, peers, participants, volunteers, and other stakeholders.

  • Act as team leader for shift and support peer mentors (recovery coaches) in their decision-making regarding service provision.

  • Conduct on-site skills building groups, workshops, and wellness activities using evidence-based criteria and best practice interventions.

  • Represent the organization at meetings and speaking engagements, as requested, to promote the program as well as attending required trainings.

  • Participate in staff meetings and case conferences and work as a team player.

  • Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values.

  • Other duties as assigned.

Requirements

QUALIFICATIONS:

  • CASAC or college degree in Social Work or Counseling required.

  • At least 1 year of experience providing counseling services to individuals experiencing substance abuse or mental health issues.

  • Strong assessment skills, care planning, and computer literacy.

  • Effective communication in written, verbal, an interpersonal communication skill. Ability to organize thoughts in a clear and concise manner.

  • Ability to work efficiently as part of an integrated team in a fast-paced environment while displaying sensitivity to the complex needs of the individuals we serve.
