About the Department Lakewood Pines and Lakewood Manor are licensed assisted living facilities. Lakewood Pines has 43 apartments, and Lakewood Manor has 26 apartments. Mission for Position As an LPN in Assisted Living, our mission is to provide compassionate, person-centered care that enhances the quality of life for each resident. Through skilled nursing support, clear communication, and dedication to promoting dignity and independence, we strive to create a safe, comfortable, and nurturing environment where residents feel respected and valued. Our focus is on delivering excellent care while fostering trust and building meaningful relationships with residents, their families, and the broader care team. Duties & Responsibilities Provide clinical assistance for Lakewood Pines and Manor locations to ensure resident safety and care. Ensure team members adhere to LHS clinical standards while delivering care. Participate in care conferences to share insights and recommendations for resident care enhancement. Educate and support new employees on LHS Assisted Living procedures. Stay informed on gerontology nursing practices and skill development. Administer and manage medications according to resident plans of care and orders from physicians. Monitor residents' health conditions, consulting with the RN when necessary. Document resident interactions, conditions, and care updates accurately. Position Type Full-time position, working 64 hours per pay period. 4-week days, with occasional weekend needs. Qualifications Qualifications Graduate of an accredited school of nursing. Current LPN licensure in the State of Minnesota. Senior care experience is preferred. Certifications: CPR/BLS.
