About the Department Our Care Center team focuses on providing exceptional long-term care to residents. We collaborate with interdisciplinary team members to ensure residents receive quality, personalized care that aligns with their unique needs and preferences. Mission for Position Lead and coordinate the delivery of high-quality, person-centered care while supporting and supervising the clinical care team to meet the needs of residents in a long-term care setting. Duties & Responsibilities Conduct rounds to monitor resident safety, infection control, and quality of care. Respond to emergent situations requiring RN intervention. Coordinate resident care plans and ensure appropriate care delivery. Communicate updates and changes in resident conditions to families and the care team. Supervise LPNs, TMAs, and CRAs to ensure compliance with care standards and policies. Adjust staffing based on unit needs and resident care requirements. Complete and review documentation, including interventions, care plans, and physician orders. Assist with the admission and discharge process, ensuring all assessments and care plans are completed. Monitor quality indicators such as pressure ulcers, psychotropic medication use, and fall prevention. Participate in root cause analysis and quality improvement initiatives. Ensure compliance with OSHA standards and long-term care regulatory requirements. Position Type Full-Time, Monday-Friday 8:30-5, 40 hours per week. Additional pay for on-call, weekend shifts, and holidays as applicable. Qualifications Qualifications Graduate of an accredited school of nursing. Current RN licensure in the State of Minnesota. Experience in leadership preferred. Experience in long-term, transitional, and hospice care preferred.
