Clinical Care Coach (LPN/RN) - PRNBluegrass Transitional Care: Stanford, KY Coming home from the hospital or facing chronic health conditions can be challenging. The staff will develop a wellness coaching relationship with clients and assist the client towards better health by providing support, encouragement, education, and resources when needed. RequirementsIs a graduate of an accredited school of professional (registered) nurses, or practical nurse or vocational nursing program.Is currently licensed as an RN or LPN through the State Board of Nursing.Minimum of 1-2 years’ experience in home health, health care or community-based organization.Strong communication and organizational skills.Knowledge of medical terminology.Strong education/teaching background.Ability to be flexible and adjust practice based on client population being served.Experience in problem solving, managing complex problem, and developing solutions.Strong customer service skills and the ability to work independently.Ability to exercise judgment and discretion in confidential matters.Must be able to make home visits to patients in a variety of settings; mobile homes, apartments, homes, nursing facilities and condominiums that may not have elevators.Must be able to drive for extended periods of time with no ill effects.Must be flexible, good written and oral communication skills.Must have computer access (PRN only) and excellent computer skills (ALL).Must have the ability to give and receive constructive criticism.Employees will be expected to comply with Bluegrass Care Navigators Employee Health, infectious disease, and vaccination policies.Must be a licensed driver with automobile that is insured in accordance with state and/or BCN requirements and is in good working order.
